摘要
目的探讨低温等离子射频消融术与臭氧注射术治疗腰椎间盘突出症(LDH)的疗效及对患者生活质量的影响。方法选取2015年12月至2017年2月在本院接受治疗的LDH患者240例,采用随机数字表法将所有患者分为低温等离子组(采用低温等离子射频消融术治疗)和臭氧注射组(采用臭氧注射术治疗)各120例。比较两组患者的手术指标、并发症情况以及不同时间点的视觉模拟疼痛量表(VAS)得分、麻木和肌力指标、世界卫生组织生活质量简化量表(WHOQOLBREF)得分,并比较两组患者不同影像学分型的治疗效果。结果臭氧注射组的手术时间、住院时间和治疗费用均小于低温等离子组(P <0. 05)。两组不同时间点的VAS评分、麻木评分比较差异无统计学意义(P> 0. 05);术后3个月低温等离子组的挺腹时间和仰卧蜷腹时间高于臭氧注射组(P <0. 05);术后3、6个月低温等离子组外侧型的优良率高于臭氧注射组(P <0. 05);术后1周、术后1、3、6个月低温等离子组的WHOQOL-BREF量表生理领域得分、心理领域得分和总得分显著高于臭氧注射组(P <0. 05)。两组的并发症发生率比较差异无统计学意义(P> 0. 05)。结论低温等离子射频消融术治疗外侧型LDH效果更佳,且能够更明显地提高患者的腰背肌肌力和腰腹肌力,改善患者的生活质量,但手术时间和住院时间较久,治疗费用更高。
Objective To investigate the curative effect of low temperature plasma radiofrequency ablation and ozone injection in the treatment of lumbar disc herniation (LDH)and its effect on the quality of life of patients.Methods 240 patients with LDH who were treated in Xining First People's Hospital from December 2015 to February 2017 were enrolled in the study.The patients were divided into low temperature plasma group (low temperature plasma radiofrequency ablation therapy)and ozone injection group (ozone injection therapy)by random number table method,with 120 cases in each group.The operative indexes, complications,and visual analogue scale (VAS)score at different time points,numbness and muscle strength indexes and WHO quality of life scale (WHOQOL-BREF)score were compared between the two groups.The clinical efficacy was compared between patients with different image credit type in two groups. Results The operation time,hospitalization time and the cost of treatment in the ozone injection group were less than that of the low temperature plasma group (P <0.05).There was no significant difference in VAS score and numbness score between the two groups at different time points (P >0.05).The abdominal time and the supine crouch time in the low temperature plasma group in 3 months after operation were longer than those in the ozone injection group (P <0.05).The excellent rate of the lateral type in low temperature plasma group in 3months and 6 months after operation was higher than that in the ozone injection group (P < 0.05).The physiological field score,psychological field score and total score in WHOQOL-BREF of low temperature plasma group in 1month,3months and 6 months after operation were significantly higher than those of ozone injection group (P <0.05).There was no significant difference in the incidence of complications between the two groups (P >0.05).Conclusions Low temperature plasma radiofrequency ablation has better effect in the treatment of lateral LDH,and it can improve lumbar and dorsal muscle strength a
