摘要
目的 研究实验性动静脉瘘的血液动力学特征。方法 2 4只成年兔分为A、B两组 ,每组 12只 ,均形成颈部动静脉瘘 ,并于瘘形成后 3个月时将瘘口结扎。A组于瘘形成后 1~ 3个月及瘘结扎后 1~ 3个月每月均行彩色多谱勒检测 ,B组则行瘘血管直径测量 ,以观察其血液动力学变化。结果 A组 10只兔的动静脉瘘于瘘形成后 3个月仍通畅 ,血管直径呈进行性增大 ;B组 9只兔的动静脉瘘于瘘形成后 3月内通畅 ,血管直径呈进行性增大 ;余 5只兔 (A组 2只 ,B组 3只 )的动静脉瘘则自行关闭。瘘结扎后 ,血流流速明显下降 ,而血管直径变化不明显。结论 动静脉瘘形成后 ,多数呈进行性发展 ,瘘动静脉持续性扩张 ;瘘口结扎后 ,虽血流流速明显下降 ,但血管扩张状态难以改善。
Objective To study the hemodynamic features of experimental AVFs.Methods Twenty-four rabbits were subdivided into Group A (n=12) and Group B (n=12);experimental neck AVFs were surgically constructed in all 24 rabbits,and the fistulas were ligated three months after formation.The rabbits in Group A experienced Color Doppler investigation once a month during patency and occlusion of AVFs(6 months in all),and the rabbits in Group B experienced vascular diameter measurement in a similar time course.The hemodynamic data was statistically analyzed.Results AVFs in ten rabbits of Group A were clear three months after formation and the flow velocity was continually increased;nine of Group B AVFs were clear within three months and the size of the fitulous vessels were instantly enlarged.The other five AVFs(2 of Group A and 3 of Group B)experienced spontaneous occlusion within three months.After surgical ligation of the fistulas,the flow velocity was apparently decreased,whereas the diameter of the vessels wasn't accordingly decreased.Conclusion Owing to the hemodynamic consequences,most AVFs has a trend of progressive advancement.After treatment,the flow velocity would be apparently decreased,whereas the size of vessels would not be accordingly reduced.
