摘要
目的 当存在椎管发育性狭窄或退行性改变时 ,外伤可致无骨折脱位的颈脊髓损伤 ,本文的目的是阐明该病发病机制、MRI检查的价值。方法 对 2 4例无骨折脱位的颈脊髓损伤病人进行回顾性分析。结果 大多数病人X线检查存在颈椎的退行性变化 ,CT检查发现后纵韧带骨化和发育性狭窄 ,MRI扫描示髓内T2 像高信号。结论 颈椎的退行性改变和颈椎管的发育性狭窄是发病的病理基础 ,在急性期MRI扫描对了解脊髓受伤情况、机制是有用的 ,对预后判断及治疗有一定指导意义 ,多数病人尽管无骨折脱位 ,但仍存在脊髓受压 。
Objectives: The cervical spinal cord can sustain traumatic injury without fracture or dislocation when preexisting degenerative changes of the cervical spine or developmental narrowing of the spinal canal.The purpose of this article was to understand the mechanisms of this disease and the value of MRI for it.Methods:24 patients with acute traumatic cervical spinal cord injury without fracture and dislocation were retrospective reviewed.Results: X-ray photo demonstrated degenerative changes of the cervical spine,CT scan found ossification of the posterior longitudinal ligament and developmental narrowing of the spinal canal,and intramedullary hyperintensity on T 2-weighted MRI in the majority of the patients.Conclusions:Degenerative changes of the cervical spine and developmental narrowing of the spinal canal are important preexisting factors.In the acute stage MRI is useful to understand the degree and mechanisms of spinal cord injury,provided data of reference for the therapeutic evaluation of treatment and the prognosis evaluation of patient's recovery.A significant number of the patients were found to have spinal cord compression despite the absence of bone injuries of the spinal column indicates that surgical treatment of this type of injury are necessary.
