成人左心辅助心电触发方式下辅助延迟时间的探讨 被引量:1

摘要 本文旨在寻找一个经验公式以有效地指导成人左心辅助装置触发延迟时间。目的 :以R Ao代表心电QRS波峰点与主动脉瓣关闭结束的间隔时间 ,以RR代表RR间期 ,找出RR与R Ao的相关方程R Ao =A·(RR) n+B。通过加权平均法计算RR的平均值 ,再求出R Ao预期值 ,对R Ao做安全较正后 ,以此做为辅助的延迟时间。方法 :选择 14岁以上行升主动脉导管的成人患者 ,经筛选 ,测量 877例心导管记录图中心电部分的RR间期以及与此心电对应的主动脉压力波形 ,测量主动脉下降波切迹与QRS波起始点间的间隔 ,以R Ao为因变量 ,以 (RR) n 为自变量 ,绘散点图 ,找出相关模型 ,并求出相关方程。结论 :当心率小于 12 0次 分时 ,相关系数为 0 5 0 6 6 ,回归系数为 - 0 0 948,P值为 1 96 8E 5 8;截距为 0 6 5 5 ,P值为 8 0 97E 10 9,相关方程为 :R Ao =(- 0 0 948)RR1 3 +0 6 5 5 1。 The aim of our tests was to find a formula to calculate the LVAD time delay.objects:While RR represents the duration of RR period and R-Ao represents the duration of the period between the peak point of QRS and the point of aortic valve closing,we tried to find a formula correlating RR and R-Ao like:R-Ao=A·(RR) n+B(A and B are constants).First,we calculate RR according to weighted average method,and then calculated the expected R-Ao value according to the above-mentioned formula.After safety adjustment,R-Ao was used as the assistance time delay.Methods:We selected 877 adult pediatric patients who had received left heart catheter and made sure that these patients were not arrhythmic.At the ECG part of catheterogram,we measured RR,and the corresponding aortic pressure waves.We measured the period between the aortic desconding wave intersection point and the beginning point of QRS wave.We ploted graphs of scattered points with R-Ao as the dependent variable and (RR) n as the independent variable.We found a correlation model and calculated the arguments A and B of R-Ao=A·(RR) n+B.Results:The relation between (RR) 1/3 and R-Ao is the most significant.When heart rate is less 120bpm,the coefficient of correlation was 0 5066,the regression coefficient was -0 0948,and P=1 968E-58,the intercepted distance was 0 655,P value was 8 097E-109,and the correlation equation was R-Ao=(-0 0948)(RR) 1/3 +B.
