摘要
目的 探讨胰岛素抵抗 (IR)与冠心病患者冠状动脉狭窄程度及范围的关系。方法 纳入冠状动脉造影后的患者 97例 ,男、女各分一组 ,男 63例 ,女 3 4例 ,再将冠状动脉狭窄≥ 75 %者分为冠心病组 ,狭窄 <2 5 %者为对照组 ,所有患者均抽血查胰岛素、糖耐量试验、胆固醇、甘油三酯、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、载脂蛋白A、载脂蛋白B水平 ,冠状动脉的狭窄程度用造影图像处理系统测量 ,再据病变损害的程度评分。用统计学方法分析控制其他易患因素影响后IR与冠心病的定性关系和IR与冠状动脉狭窄程度及范围的定量关系。结果 经偏相关分析发现 ,男、女冠心病患者均与IR有关 ,IR独立于其他冠心病易患因素。多元回归分析发现 ,无论男、女IR均与冠状动脉狭窄的病变程度呈正相关 (P <0 0 0 5 )。结论 IR可作为预测冠心病严重程度的参考指标之一。
Objective To investigate the relationship between insulin resistance (IR) and severity of coronary artery stenosis. Methods 97 patients who underwent coronary angiography were included in the study and divided into two groups according to gender: male group (n=63) and female group(n=34).Each group was further divided into 2 subgroups according to severity of coronary artery stenosis: patients with coronary stenosis ≥75% were included in the subgroup of coronary artery disease (CAD)and patients with normal coronary artery or stenosis <25% were used as controls. Plasma insulin (IS), blood glucose (BG), total cholesterol (TC), triglycerides (TG), lower density lipoprotein cholesterol (LDL-C),high density lipoprotein cholesterol (HDL-C), apolipoprotein A, apolipoprotein B were determined in all patients. Coronary artery stenosis was measured by angiographic imaging manipulation system .The severity of CAD was quantified in a modified Gensini score. After the influences from other risk factors of CAD were controlled by statistical method ,the relationship between IR and severity of CAD was analyzed.Results The relationship between CAD and IR was confirmed. Partial correlation analysis showed that IR was one of the independent risk factors for CAD. Multiple regression analysis indicated that IR was positively correlated with CAD in both gender (P<0.000 5). Conclusion IR may be one of the predictors for severity of CAD.
