治疗胸腰椎结核两种前路内固定术的比较 被引量:4

摘要 目的 脊柱胸腰椎结核手术方法较多 ,本文介绍 Kaneda和 Z- plate两种前路内固定的临床应用 ,着重对其进行比较 ,为临床选择最佳手术方案提供参考依据。方法 总结 38例胸腰椎结核前路手术经验 ,其中 2 3例进行 Kaneda术 ,15例行 Z- plate术。结果 本组 38例获 8- 41个月随访 ,均未见固定物松动或断裂等现象 ,疗效满意。结论  Kaneda和 Z- plate内固定术均可为胸腰段脊柱重建提供更好的三维稳定性和可靠性。治疗胸腰椎结核 Z- plate术较 Kaneda术操作简便、手术时间短、术中出血少及并发症低 ,发展前景大。但 Z- Objective A Lots of surgical methods for treatment tuberculosis(TB) of thoraco lumbar spine,at present the author introduce and compare two kinds of anterior fixation techniques,Kaneda and Z plate,in order to furnish evidence for clinical operative plan.Methods 38 cases of thoraco lumbar spine TB treated by anterior fixation technique(including 23 Kaneda device and 15 Z plate system) were followed up and summarized.Results 38 cases were followed up from 8—41 months and the treatment result was clinically satisfied.No complication caused by implants has been found.Conclusion Both Kaneda device and Z plate system can supply better three dimensional stabilization and reliance for thoraco lumbar spine reconstruction.Z plate system has good development because the comparing results have show that it has the advantage of simple assembling,less operative time,less blood lost and less complication,but high price is a handicap to clinical application.
