摘要
探讨一期手术治疗双侧上尿路结石致急性肾功能衰竭的治疗效果。方法 :本组 2 8例均行一期手术 ,包括肾盂切开取石 31侧 ,肾盂、肾实质联合切开取石 9侧 ,输尿管切开取石 16侧 ,术中留置硅胶支架管 5 1侧 ,留置双J管内引流 5侧 ,其中 3例术前行血液透析 1～ 4次。结果 :术后 1例单侧出现尿外渗切口感染 ,2 7例术后出现多尿期 ,1例经再次血透后出现多尿期 ,无 1例死亡。 17例术后 7～ 10d复查BUN、Cr恢复正常 ,11例明显下降。结论 :双侧上尿路结石一期手术取石解除梗阻能有效挽救肾功能。
Objective:To explore the clinical effect of one stage open operation on ilateral upper urinary tract stones with acute renal failure (ARF).Methods:Open operation in one stage on both sides were performed in 28 cases of bilateral upper urinary tract stone with ARF,including 31 pyelolithotomies,9 nephrotomies combined with pyelotomies,16 urterolithotomies.Silastic catheters were used as internal stents in 51 sides,doube J catheters were used for internal drainage in 5 sides.Hemodialysis were performed one to four times preopratively in 3 cases.Results:28 cases prectessed hydruric periods (including one after hemodialysis)postopratively.Renal functions got to nomal level in 17 cases and improved abruptly in 11 cases 7～10 days after operation.There's no mortality in our cases.One case had incision infection.Conclusions:The one stage open operation for bilateral upper tract stones was effective and reliable.
