微侵袭立体定向活检方法的研究 被引量:13

Clinical study of minimally invasive stereotactic biopsy for brain lesions

摘要 目的 评价微侵袭立体定向活检术在确定脑深部或主要功能区病变的病理组织学诊断中的作用。方法 在近 10年立体定向手术经验的基础上 ,将微侵袭观点引入立体定向活检手术并对15 5例脑深部或主要功能区病变进行了活检手术研究。其中男 89例 ,女 6 6例 ,年龄自 6 5~ 6 8岁 ,平均 31 3岁。行CT引导的立体定向活检 43例 ,MRI引导 112例。结果 组织病理学诊断结果为脑肿瘤118例 ,炎性疾病 15例 ,寄生虫或囊肿样病变 10例 ,放射性坏死 6例 ,脑变性性疾病 3例 ,3例未能获得阳性病理学诊断 ,总阳性诊断率为 98 1%。共发生并发症 4例。无死亡发生。结论 立体定向活检是一种明确颅内占位病变的组织学诊断的可靠方法 ,并能为临床治疗方法的选择提供依据。 Objective To investigate the role of minimally invasive stereotactic biopsy in making correct pathological diagnosis of brain lesions. Methods On the basis of ten years experience of stereotactic operations, from October, 1996 to December 2000, minimally invasive stereotactic biopsy had been performed in 155 patients with intracerebral lesions which were deep seated or located in brain functional area. There were 89 males and 66 females with ages ranged from 6 5 to 68 years (average 31 3 years). CT guided biopsy was performed in 43 cases and MRI guided in 112.Results The brain tumors were diagnosed pathologically in 118 cases, inflammatory processes in 15, parasites or cystic lesions in 10, radioactive necrosis in 6 and degeneration of the nerve system in 3. Non diagnostic cases were in 3. The all positive biopsy rate was 98 1%. The complications after biopsy were found in 4 cases. Neither severe complication or death related to the operation was found.Conclusions The results suggest that stereotactic biopsy is a reliable method for histopathological diagnosis of brain lesions and it is also of great help in selecting appropriate management. Minimally invasive technique can increase the positive biopsy rate and decrease the complication rate.
