自制镍钛合金室间隔缺损封堵器的生物相容性研究 被引量:7

摘要 目的 :观察自制室间隔缺损封堵器植入动物体内后的组织学反应。方法 :选用杂种猪 6只 ,体质量 15~ 2 0 kg。应用硫喷妥钠静脉麻醉 ,开胸暴露心脏 ,在右心室近心尖处穿刺右心室壁、室间隔 ,通过 10 F鞘管将自制镍钛合金室间隔缺损封堵器置入室间隔处。观察 45~ 12 0 d。结果 :6只猪中 1只因术中出血死亡 ,另 1只猪术后 8h死亡 ;4只猪存活。术后 45 d和 6 0d各处死 1只猪 ,12 0 d处死 2只 ,大体标本显示封堵器表面覆盖一层新生内膜 ,光镜观察新生内膜表面为一单层扁平细胞所覆盖。在早期 ,封堵器植入处有炎症细胞浸润 ;后期炎症反应消失 ,为纤维组织所代替 ,表现为组织损伤后的修复过程。肺、肝脏、脾脏、肾脏、大小肠无血栓栓塞现象。 结论 :自制镍钛合金室间隔缺损封堵器有很好的生物相容性。 Objective: To evaluate the biocompatibility of self made nitinol alloy ventricular septal defect occluder. Methods: Six nitinol alloy ventricular septal defect occluder were implanted in the ventricular septum by catheter in 6 normal anaesthetized open chest pigs, and the animals were observed for 45 to 120 d(2 animals). Results: One deaths resulted from hemorrhage and another from embolization of occluder in abdominal aorta during the placement procedure. Successful placement of the occluder was achieved in 4 animals. Four animals were killed at 45, 60 and 120 d. Postmortem gross and microscopic examination of 4 devices 45 to 120 d after placement showed that both the right and left ventricular discs of the occluder were completely covered by a smooth, shiny, glistening thin layer of neoendocardium, and the surface of neoendocardium was covered by a monolayer of endothelium like cells. The inflammatory infiltrate around the occluder was found at 45 d, and disappeared and fibrosis formed at 120 d. These appeared as a repair process of the injury. Embolization in lung,liver, spleen, kidney, intestinal and colon were not found. Conclusion: These suggest that the self made nitinol alloy ventricular septal defect occluder has good biocompatibility. [
