糖皮质激素对呼吸道合胞病毒反复感染小鼠肺组织病理改变的影响 被引量:47

Influence of glucocorticoids upon pathological changes of pulmonary tissue with RSV reinfection

摘要 目的 探讨地塞米松 (Dex)对呼吸道合胞病毒 (RSV)反复感染小鼠肺病理改变的影响。方法 通过RSV两次感染小鼠 ,用光镜、电镜观察肺组织形态 ,对RSV肺炎 +地塞米松 (Dex)处理组及肺炎对照组进行对比研究。结果 ①首次感染RSV肺炎组小鼠肺泡壁破坏显著 ,胞质内有较多RSV颗粒 ;肺间质淋巴细胞浸润明显 ,肺泡壁增厚 ,第 7d达高峰 ,第 9d后逐渐好转。Dex组肺泡壁破坏轻 ,RSV颗粒存在时间长 ;细胞浸润等炎症反应第 9d达高峰 ,第 12d无缓解。②再次感染RSV鼠 ,Dex组及对照组较首次感染有早而重的肺泡炎症。但Dex组较对照组炎症反应持续时间长 ,尤其在第 12d时Ⅱ级以上炎症仍占 6 7% ,胞质内有RSV颗粒。对照组再次感染肺泡壁的损害较首次轻 ,无RSV颗粒 ,炎症反应重 ,恢复快。结论 Dex对RSV首次及再次感染鼠均有减轻炎症反应的作用 ,但使炎症高峰期推迟 ,病毒清除能力减低 ,病程明显延长。 ObjectiveTo explore the influence of Dexamethasone(Dex)upon pathological changes of pulmonary tissue with RSV reinfection.MethodsBy using electron microscope and light microscope,morphological changes of pulmonary tissue of rats with RSV reinfection had been observed between RSV pneumonia plus Dex treatment group and pneumonia control group.Results①Remarkable lesion of alveoli wall had been observed in rats with primary RSV pneumonia,and many RSV granules could be discerned in cytoplasma;there was obvious infiltration of lymphocytes in pulmonary interstitium and thickening of alveoli wall.It reached the summit at the 7th day,and improved gradually after 9 days.By contrast,we observed a slight lesion of alveoli wall in Dex treatment group and prolonged existence of RSV granules.Inflammatory reactions reached the summit at the 9th day and there was no obvious improvement at the 12th day.②We observed more severe and advanced pulmonary inammation than that of the primary inflection group in Dex group and control group with RSV reinfection,but the inflammatory reactions of Dex group lasted longer than that of the control group,especially the percenter of Ⅱ to inflammation was still 67% at the 12th day,and RSV granules in cytoplasma.We observed a slighter lesion of alveoli wall than that of the primary infection in control group with RSV re-infection,no RSV granules,inflammatory reactions were more severe and recovery period was shorter.ConclusionThe function of Dex to alleviate the inflammatory reactions in RSV primary infection group and reinfection group has been shown,but it can result in the delayed arrival of inflammation summit and disabled clearance of virus,so the course of disease is prolonged obviously.It has suggested that the usage of glucocorticoids should be cut down as much as possible in treatment of RSV pneumonia.
