摘要
目的 探讨血清细胞外基质作为风湿性心脏病 (风心病 )心肌纤维化时血清标志物的临床价值。方法 用放射免疫测定法 (RIA)检测 30例风心病病人和 2 0名健康成人血清中I型前胶原 (PCI)、III型胶原 (PCIII)及层粘连蛋白 (LN)的水平 ,同时用免疫组化及图像分析方法检测风心病心肌中I、III型胶原及LN ,对血清及心肌中细胞外基质水平作对比分析。结果 风心病血清中PCI、PCIII及LN均较正常成人明显增高 (P <0 0 1) ,且均与心肌纤维化面积及心功能分级呈正相关 ,并分别与心肌中相应成分的表达一致。结论 血清细胞外基质可作为风心病心肌纤维化时的血清标志物 。
Objective: To investigate the clinical values of serum extracellular matrix (ECM) as an useful index of rheumatic myocardial fibrosis. Methods: Circulating levels of procollagen type I (PC?I), procollagen type III (PC?III) and laminin (LN) were measured by radioimmunoassay in 30 patients with rheumatic heart disease (RHD) and 20 healthy adults. Amount of type I, III collagen and laminin in myocardium were examined by immunohistochemical staining and imaging analysis. The correlations between serum ECM and myocardial counterpart were analyzed. Results: The levels of serum ECM in patients with RHD were significantly higher than that in healthy adults (P<0 01) and correlated positively with the area of myocardial fibrosis and the NYHA cardiac function class. Serum PC?I, PC?III and LN were significantly correlated with the myocardial area fractions of their tissue analogues. Conclusion: The serum level of ECM can be used to reflect the degree of cardiac fibrosis in patients with RHD.
