摘要
目的 探讨倍频Nd∶YAG激光周边虹膜成形术后的虹膜组织病理学改变及作用机制。方法 采用倍频Nd∶YAG激光对猴眼的周边部虹膜行激光周边虹膜成形术 ,通过裂隙灯显微镜 ,光镜及扫描电镜观察术后1、3个月的虹膜组织病理学改变。结果 术后 1、3个月的光镜和扫描电镜显示光凝处的虹膜收缩纹和不同程度的纤维细胞及胶原纤维增生。结论 证实了激光周边虹膜成形术的作用机制 :(1)虹膜组织内黑色素细胞及胶原蛋白吸收光能并转变为热能引起光凝固反应 ;光凝部位虹膜表面的成纤维细胞增生及胶原蛋白形成 ,导致产生瘢痕。 (2 )该术式可减轻虹膜根部膨隆 ,增宽前房角 ,增加房水引流。
Objective To investigate histopathological changes of iris after frequency doubled Nd∶YAG laser peripheral iridoplasty and its mechanisms. Methods Frequency doubled Nd∶YAG laser peripheral iridoplasty was performed on monkey iris. Through slit lamp microscopic examination, light microscopic examination and scanning electron microscopic examinations, histopathological changes of the iris one month and three months postoperatively were observed. Results Iris contraction, different degrees of fibroblasts and collagen proliferation was shown in the area of laser burns. Conclusions The results verify two mechanisms of this technique. First, melanocytes and collagen of the iris absorb laser light and transfer it to heat causing photocoagulation; fibroblasts and collagen proliferation causing scar formation in the area of laser burns. Second, it can relieve iris bombe, widen anterior chamber angle and increase aqueous outflow. The results suggest that this technique might be used in clinical practice for primary angle closure glaucoma.
