摘要 目的 分析大肠癌组织的激光诱发自体荧光 (LIAF)光谱规律及荧光产生机制。方法 以氮分子激光器和光学多通道分析仪 (OMA)Ⅲ系统 ,自癌灶中心至正常组织分别连续检测 19例大肠癌手术标本LIAF光谱 ,在激光共聚焦扫描显微镜下分别检测正常结肠组织及癌组织的荧光强度。结果 LIAF连续光谱主峰波长由正常组织经癌旁至癌组织逐渐向红光侧移动 ,癌组织平均移动约 5nm ,与正常组织比P <0 0 0 1;主峰强度亦依次降低 ,癌组织与正常组织相比 ,平均减低 75 18 90 ,P <0 0 0 0 1。激光共聚焦扫描显微镜提示正常组织粘膜下层荧光 (10 5 79)最强 ,肌层荧光 (85 32 )次之 ,而粘膜层 (4 7 74)与癌组织 (4 4 2 1)的荧光最弱。结论 结肠组织LIAF主要来源于粘膜下组织 ,癌组织的低荧光与其粘膜厚度增加可能有关。 Objective To assess the feature of laser induced autofluorescence(LIAF) spectra for detection of colorectal carcinoma, and study the regular pattern and mechanism of the production of spectra. Methods Tissue autofluorescence was excited by a nitrogen laser (337 nm), and then the fluorescence emission was collected by the same quartz optical fiber. The LIAF spectra was analyzed using an optical multichannel analyzer (OMAⅢ). The 19 specimens from patients with colorectal carcinoma undergoing colorectal resection were analyzed sequentially by LIAF from cancer foci to normal tissue mucosa more than 5 cm away from the foci. The same specimens were also frozen and sliced longitudinally from serous to mucosa surface, comprising mucosa, submucosa and muscuaris, for the analysis of confocal laser scanning microscopy. Results (1) The main peak intensity of sequential LIAF spectra was gradually decreasing in order from normal tissue, adjoining tissue to cancer tissue, the mean peak intensity decreased from the normal tissue to cancer tissue was 7518 90, P<0 0001; (2) The wavelength of main peak of sequential LIAF spectra was gradually shifted to the red fluorescence in order from normal tissue, adjoining tissue to cancer tissue, the mean wavelength shifted to the red fluorescence was 5 nm from the normal tissue to cancer tissue, P<0 001; (3) In term of the fluorescence intensity, submucosa tissue from normalcolon tissue was the strongest(105 79), followed by muscuaris(85 32) and mucosa(47 74) which was paralleled to cancer tissue(44 21), P>0 05. Conclusions Our findings (1) demonstrated that the LIAF spectroscopic features of colorectal cancer tissues are markedly different from those of the corresponding normal tissue; (2) The submucosa plays an important role in the LIAF spectra in the normal tissue, the change from mucosa thickness may be related to being lower of LIAF intensity from cancer tissue. This method may be helpful to detect precancerous lesions.
