摘要
目的 :探讨上尿路肿瘤 (U U TT)伴发膀胱癌的临床病理特征。方法 :回顾性分析 81例 UU TT患者的临床病理资料 ,其中 35例伴发膀胱癌 ,分为膀胱癌先发于 UU TT6例 (第 1组 ) ,膀胱癌和 U UTT同时发现 12例 (第 2组 ) ,膀胱癌继发于 UU TT17例 (第 3组 )。结果 :多器官性 UU TT患者的膀胱癌发生率为 75 % ,高于单发性肿瘤者的 40 % (P <0 .0 5 ) ;其生存率与是否伴发膀胱癌无关。 UU TT的分期及分级在第 2组高于第 1、3组(P <0 .0 5 ) ,第 2组的生存率低于第 1、3组 (P <0 .0 5 )。结论 :UU TT伴发膀胱癌的生物学特征与膀胱癌发生的时间有关 ;UU TT同时伴发膀胱癌的患者预后较差 ,需密切随访 。
Purpose:To analyze the clinicopathological features of bladder cancer associated with upper urinary tract tumor (UUTT).Methods:Among 81 patients with primary UUTT, 35 also had bladder cancer. These cases were divided into three groups, bladder tumor preceding UUTT, bladder tumor found simultaneously with UUTT, and bladder tumor following UUTT.Results:The incidence rate for patients with multiple site tumor was 75% compared to 40% for those who had single site tumor(P< 0.05 .There was no difference between survival rates of the patients with and without associated bladder cancer. The incidence of high stage or high grade UUTT in group 2 was higher than that in group 1 and 3 (P< 0.05 ); and the survival rate in group 2 was lower than in group 1 and 3 (P< 0.05 ); furthermore, the survival rate of patients in group 2 was lower than that of all other patients (P< 0.05 ).Conclusions:Bladder cancer associated with UUTT has a different biological character depending on the timing of tumor appearance, patients with concurrent bladder tumors had a worse prognosis.
