脑干听觉诱发电位对椎-基底动脉缺血发作的诊断价值 被引量:1

摘要 目的 探讨脑干诱发电位 (BAEP)对椎 基底动脉缺血发作 (VBI)的诊断价值。方法 对临床诊断VBI的32例患者做BAEP检查 ,观察Ⅰ ,Ⅲ ,Ⅴ波的潜伏期 (PL)、波形分化情况、Ⅰ~Ⅲ ,Ⅲ~Ⅴ ,Ⅰ~Ⅴ波的峰间期 (IPL)、两耳测量值的差值 (ILD)以及I/V波幅比值。结果  32例中有 2 8例 ( 87.5 % )异常。其中前庭周围异常 11例 ( 34 .4% ) ,主要表现为Ⅰ波分化不良或波形消失 ,PL延长 ,相应ILD增大。脑干下部异常 13例 ( 40 .6% ) ,表现为Ⅲ波分化不良或波形消失 ,Ⅲ波PL ,Ⅰ~Ⅲ波IPL延长 ,相应ILD值增大。脑干上部异常 4例 ( 12 .5 % ) ,为Ⅴ波分化不良或波形消失 ,Ⅴ波PL ,Ⅲ~Ⅴ波IPL延长 ,相应ILD值增大。结论 BAEP应是一个检测VBI比较敏感的方法 ,当脑干损害时 ,只要电位活动发生改变 ,BAEP就有表现。还可根据波幅的变化 ,判断神经传导通路的损害部位及受累血管。 Objective To investigate the diagnose value of the brainstem auditory evoked potential (BAEP) and vertebra-basal artery ischemia attack (VBI).Method 32 patients with VBI that clinical examination was taken BAEP.To observe the peak latency (PL) of waves Ⅰ,Ⅲ and Ⅴ,form of waves,interpeak latency (IPL) of wavesⅠ~Ⅲ,Ⅲ~ⅤandⅠ~Ⅴ,interear latency difference (ILD) of two ears and the ratio ofⅠ/Ⅴ.Result There are 28 abnormal cases in this group.Of them,11 cases vestibule peripheral nervous system are unusual,the waves Ⅰ split is bad and the form of it is disappear,PL is prolong,ILD is increase correspondingly.13 cases the lower part of the brainstem is abnormal,the waves Ⅲ split is bad and the form of it is disappear,PL and IPL of the waves Ⅰ~Ⅲ are prolong,ILD is increase correspondingly.4 cases the supper part of the brainstem is abnormal,the waves Ⅴ split is bad and the form of it is disappear,PL and IPL of the waves Ⅲ~Ⅴ are prolong,ILD is increase correspondingly.Conclusion BAEP ought to be a more sensitive method of determine to VBI.When the brainstem is damage,its electric potential changes will take place and be recorded by BAEP.Moreover,on the basis of the change of amplitude,thus judging the location of the injury and inferring involved the vessel.
