摘要
为探索乙肝病毒 (HBV)S基因 (HBVS)亚型、前C区点突变与血清乙肝病毒e(HBe)系统之间的关系 ,我们对98例血清学确诊且HBVDNA阳性的乙型肝炎患者血清采用 3′端特异性引物聚合酶链反应 (3′BS PCR)进行了1896位核苷酸的检测分析 ,同时进行片段长度多态性分析 (PCR RFLP)检测HBV亚型。结果显示 :① 98例患者中 6 3例有突变株感染 (6 4.3% ) ,野生株和突变株混合感染的检出率在HBeAg阳性组为 5 7.8% ,抗 HBe阳性组中为34 .7% ,两组间有显著性差异 (P <0 .0 5 ) ;② 98例患者中HBVS亚型Ⅰ 6 8例 (6 9.4% ) ,其中HBeAg阳性与抗HBe阳性分别为 31与 32例 ;③HBVS亚型Ⅰ与Ⅱ突变株检出率分别为 80 .9%与 2 4.0 % ,两者有极显著差异 (P <0 .0 1) ,亚型Ⅰ中野生株和突变株混合感染率为 5 7.4%。结果表明 :HBVS亚型与HBe系统无必然的因果关系 ,但HBe系统与前C区第 1896位点突变有关。HBV前C区第 1896位点突变与HBVS亚型有关 ,以亚型Ⅰ易发生。变异株的产生可能受HBVS亚型和机体免疫压力所致。
The aim of this study is to explore the relationship of HBVs gene subtype, precore mutation and HBe system.3 ′BS PCR was used to examine precore gene 1896 point mutation and PCR RFLP was used to type HBV s gene subtype in 98 serum samples of patients. All sera were HBV DNA positive. Results showed that:①The mutation of precore infection was detected in 63 out of 98 cases (64.3%).Co infection ratio of wild type and mutant type: HBeAg postitive group 57.8%. HBe Ab positive group 34.7% significant difference belween two groups (P< 0.05 ),②HBVs gene subtype I was 68 in 98 cases (69.4%),HBeAg positive was 31 cases, HBeAb positive was 32 cases,③Ratio of mutant type: subtype Ⅰ was 80.9%,subtype Ⅱ was 24.0%,significant difference between two group (P< 0.01 ),Coinfection ratio of wild type and mutant type in subtype Ⅰ was 57.4%.Results indicated that: HBV precore gene 1896 point mutation was correlation with HBe system. HBV precore gene 1896 point mutation was correlation with type of HBVs gene subtype, Gene point mutation occurred easily in subtype Ⅰ .The mutation might be associated with HBVs gene subtype and body immune press.
