摘要
目的 :分析、探讨经颅多普勒超声 (TCD)、椎动脉核磁共振血管造影 (MRA)及脑干听觉诱发电位(BAEP) 3种方法联用对椎基底动脉供血不足 (VBI)的诊断价值和意义。方法 :2 2例椎基底动脉供血不足患者分别行 TCD、MRA及 BAEP检测。结果 :TCD检测异常率为 95 .5 % (2 1/2 2 ) ,椎基底动脉异常者占总异常者的比例为80 .9% (17/2 1) ;椎动脉 MRA(包括颅内、外段 )检测异常率为 81.8% ,主要表现为单或双侧椎动脉变细、变直、迂曲、粗细不均及血流信号减低 ;BAEP检测异常率为 72 .7% (16 /2 2 ) ;TCD、MRA均异常者占 81.8% (18/2 2 ) ;TCD、BAEP均异常者占 6 8.2 % (15 /2 2 ) ;3项检查均异常者占 5 4.5 % (12 /2 2 ) ,其中结果较一致的占 75 .0 % (9/12 )。 结论 :3种方法合用从不同角度为 VBI时椎基底动脉系统异常提供了较好的非侵入性检查手段和较全面的客观依据 ,且提示 VBI时椎动脉血管变细、迂曲 ,血流速度降低 ,相应供血区神经细胞功能异常。
Objective: To analyze and investigate the value and significance of combined use of transcranial doppler(TCD), magnetic resonance angiography (MRA) and brainstem auditory evoked potential(BAEP) to the diagnosis of vertebrobasilar insufficiency (VBI). Method: 22 cases of patients with VBI were examined with TCD、MRA and BAEP respectively. Result: The total abnormality rate of TCD was 95.5% (21/22), 17 of 21(81%) patients were abnormal in vertebrobasilar artery; the abnormality rate of MRA of vertebrobasilar artery was 82%,mainly showing the unilateral or bilateral vertebrobasilar artery to be thinner, rigid,twisted and reducing of blood flow signal. The abnormality rate of BAEP was 72.7%(16/22); the abnormality rate of both TCD and MRA; TCD and BAEP were 82% and 68% respectively, 54.5% were abnormal in all three. Conclusion: The combined use of all three examinations provide abnormalities of vertebrobasilar artery system in VBI, a practically noninvasive measure and objective basis, manifestating, the vertebrobasilar artery to be thinner, rigid, twisted,reduction of velosity of blood flow and abnormality of nerve cells function in corresponding brain area.
