摘要
目的 观察博莱霉素 (BL M)肺损伤早期肺泡 型上皮细胞 (AT )的超微结构和肺表面活性物质 (PS)中磷脂(PL)及其组分的变化 ,探讨与 AT 增殖活性及修复功能的关系 .方法 BL M气管内滴注 (4 mg· m L- 1 ,5 mg· kg- 1 )制作 2 8只大鼠肺损伤模型分为 3,7,14,2 8d组 ,分别行肺组织标本电镜组织化学染色和支气管肺泡灌洗液 (BAL F)的 PS中 PL 及其组分测定并把二者作对照分析 ,观察 PS形态组成成分的质与量的改变与 AT 结构的变化间的关系 .结果 1注药后各组均可见 PS层丧失连续、均匀绒状结构 ,脱落入肺泡腔或聚集成块现象或散落于肺泡腔内 .其中以 3d组较为明显 .钌红阳性表面层的厚度与对照组比较 ,3d组较厚 ,且染色深 ,7,14d组无明显差别 ,2 8d组较淡薄 .BAL F的PS中 PL含量趋于增加 .其中磷脂酰甘油 (PG)含量 3d组升高 ,7,14,2 8d组降低 .磷脂酰肌醇 (PI)含量变化则与此相反 .2 3,7d组可见 AT 变性坏死 ,甚至崩解 ,以 3d组较明显 .AT 增生各组均可见 ,以 7d组较明显 .板层小体 3d组数量减少 ,并呈空泡状 .7d组开始增多 ,以 14,2 8d组较为明显 .33,7d组基底膜水肿、裸露 ,甚至断裂 ,炎性细胞和间隔中的成纤维细胞等渗出到肺泡腔 .14d组可见有 AT 转化为 AT ,并逐渐伸展、粘附于裸露基底膜 ,14,2 8d?
AIM To investigate the ultrastructures of rat alveolar type Ⅱ (ATⅡ) cells and Phospholipid (PL) in pulmonary surfactant (PS) and composition of PL when lungs of rats are injured by bleomycin (BLM), and to investigate the relation between PL and proliferation and reparation of ATⅡcells. METHODS Lung injury models were made by intratracheal instilling of BLM . All rats were divided into four groups, 3 d group, 7 d group, 14 d group and 28 d group, Lung preparations were stained histochemically and examined by electron microscope. PL in PS of bronchoalveolar lavage fluid (BALF) were determined, results of staining and results of determining were compared. The alternation to quality and quantity of composition of PS and the change of structure of ATⅡcells were observed. RESULTS ① Each group was found that PS layer lost continuously, appeared homogenous and chorionic, dropping in the pulmonary alveolies, collecting into masses or scattering. The change in 3 day group was more apparent. Ruthenium red attaching on pulmonary surfactant was thicker and the colour deeper in 3 d group, no difference in 7 d group and 14 d group, thinner in 28 d group. Content of PL in PS of BALF was inclined to increase. Content of phosphatidylglycerol (PG) increased in 3 d group, decreased in 7 d, 14 d, and 28 d group. The change of content of phosphatidylinositol (PI) was reversed. ② ATⅡ cells degenarated, necrotized, even disintegrated, in 3 d and 7 d group the change in 3 d group was more apparent. Proliferations of ATⅡ cells were found in each group, 7 d group was more apparent. The number of lamellar body in 3 days group decreased, appeared vacnolus. The number in 7 d group began to increase, and increased apparently in 14 d and 28 d group. ③Basement membrane was in dropsy, denuded, even fragmentated in 3 d group and 7 d group.
