心肌梗死患者血浆Ⅲ型前胶原氨基末端肽和Ⅰ型前胶原羧基末端肽含量的观察 被引量:8

Plasma concentrations of PⅢNP and PⅠCP in patients with acute myocardial infarction

摘要 目的 研究血浆Ⅲ型前胶原氨基末端肽 (PⅢNP)和Ⅰ型前胶原羧基末端肽 (PⅠCP)含量变化是否能反映心肌梗死后心肌胶原沉积。方法 应用放免法测定 3 6例AMI患者发病后 3d、7d和 2 1d血浆PⅢNP和PⅠCP含量。结果 AMI发病后 7d和 2 1d血浆PⅢNP和PⅠCP含量较健康对照 ( 2 6例 )明显升高 ;合并有高血压病史者二者升高更明显。并与同期测定的血浆血管紧张素Ⅱ和醛固酮含量有一定的相关性。结论 血浆PⅢNP和PⅠCP可反映心肌梗死后心肌间质胶原的合成和代谢 ,并可能受循环血管紧张素Ⅱ和醛固酮的影响。 Objective To investigate whether the changes of concentration of plasma aminoterminal propeptide of type Ⅲ procollagen(PⅢNP)and carboxyterminal propeptide of type Ⅰ procollagen(PⅠCP) can reflect the cardiac conllegen deposition after acute myocardial infarction. Methods Plasma PⅢNP and PⅠCP were measured by radioimmunoassay at day 3, day 7 and day 21 after onset of symptom in patients with acute myocardial infarction. Results The plasma concentrations of PⅢNP and PⅠCP were significantly increased at days 7 and days 21 after the onset of acute myocardial infarction. The patients with acute myocardial infarction who had a history of hypertension had higher levels of plasma PⅢNP and PⅠCP than those without hypertension. The plasma levels of PⅢNP and PⅠCP were significantly correlated to the plasma levels of angiotensin Ⅱ and aldosteron measured at the same time. Conclusion The measurement of plasma levels of PⅢNP and PⅠCP may reflect the synthesis and metabolism of the collagen in cardiac extracellular metrix after myocardial infarction and may be regulated by the circulating angiotensin Ⅱ and aldosteron.
