摘要
目的：研究腹腔免疫化疗在胃肠道癌术后对早期腹腔局部复发和肝转移的疗效。方法：1995年1月－1997年12月对198例行根治性手术治疗的胃肠道癌病人按年限分成腹腔化疗组122例（1995年1月-1996年12月）和腹腔免疫化疗组76例（1997年1月－1997年12月）。腹腔化疗组于术后第7天行腹腔化疗，化疗方案为FDM方案，每5d为1个疗程，每4周1个疗程，连续6个疗程，每3个月1个疗程，至术后2aq。腹腔免疫化疗组采用FDM＋IFNα-2b方案，药量及液体量同腹腔化疗组。结果：胃肠道癌根治性切除术后腹腔免疫化疗组。结果：胃肠道癌性切除术后腹腔免疫化疗组与腹腔化疗组相比，术后早期局部复发率（4／76），x^2=6.42,P<0.01，什转移率（2／76）x^2=7.22,P<0.01。3a生存率(68%、50%),x^2=12.80,P<0.01。3a以上无瘤存活例数分别为40例及32例，x^2=5.56,P<0.05。结论：胃肠道癌性术后腹腔免疫化疗可显著降低早期局部复发及肝转移，近期疗效和3a生存率明显优于腹腔化疗。
Objective:To evaluate the effect of peritoneal cavity early part recur and hepatic metastases with chemo-immunotherapy thrapies abdominal cavity after resection of gastric and intestine cancer. Methods:198 cases of gastric and intestine canaer(1995.01～1997.12)were devided into chemo-immunotherapy thrapies abdominal cavity group(76 cases 1997.01～1997.12)and chemical thrapies abdominal cavity group(122 cases 1995.01～1996.12). Every cases were treated with FDM programe after resection in seventh day, one treat programe was 5 days, every treat programe was 4 weeks, untill sixth month after operation. Every 3 months was one treat programe. until 2 years after operation. chemical thrapies vein group also. The chemo-immunotherapy thrapies abdominal cavity group was FDM+IFNα-2b programe. Results: The early part recur in chemo-immunotherapy thrapies abdominal cavity group and chemical thrapies abdominal cavity group were (4/76), χ 2=6.42, P<0.01, hepatic metastases were (2/76), χ 2=7.22 P<0.01., The 3-year survival rate in chemo-immunotherapy thrapies abdominal cavity group and in chemical thrapies abdominal group were ( 68%\,50%), χ 2=12.80, P<0.01. Recurrence-free survival in 3 years after surgery was 40 cases and 32 cases, χ 2=5.56,P<0.05. Conclusion:The chemo-immunotherapy thrapies abdominal cavity is decrease the part recur、hepatic metastases after resection, effect of treatment within one year after surgery and 3 year survival rates superior to chemical thrapies abdominal cavity from gastric and intestine cancer.
