摘要
目的 :研究修复周围神经缺损新的手术方法和验证胎儿神经、雪旺氏细胞对神经再生的作用。方法 :用80只大白鼠 ,随机分成三组 :A组自体神经移植组 ;B组单纯液氮冷冻胎儿神经组 ;C组液氮冷冻胎儿神经加自体雪旺氏细胞组。术后分别于 4,12和 2 4周取材 ,进行大体观察、组织学观察及电生理检测。临床应用胎儿神经加自体雪旺氏细胞修复周围神经缺损 32例 (4 5条 ) ,术后 1年进行神经功能评定 ,随访 1~ 3年。结果 :A ,C两组术后 4周开始再生纤维逐步增多 ,术后 2 4周再生纤维增粗并形成神经束 ,而B组再生纤维数目很少 ;复合动作电位峰值恢复率及传导速度恢复率A ,C两组恢复明显高于B组。临床应用 32例 (4 5条 )周围神经 ,按Seddon的周围神经损伤术后恢复评定标准进行评定 ,优良率为 6 5 .8%。结论 :植入自体雪旺氏细胞的胎儿神经移植是一种修复周围神经缺损的新方法 。
Objective:To explore a novel surgical technique to repair peripheral nerve defects,this article confirms the nerve regenerating effect of fetal nerve combined with autogenous Schwann cell grafts on treating peripheral nerve defects.Methods:Eighty rats were randomly divided into three groups:Group A,peripheral nerve autografts;Group B,fetal nerve grafts;Group C,fetal nerve combined with autogenous Schwan cell grafts.The nerve repair process ws observed by the gross appearance,optical and electron microscopy,image analysis,and electrophysiological examination at 4,12,24weeks after the operation.In clinic,all 45 peripheral nerve defects of 32 patients were treated by fetal nerve combined with autogenous Schwann cell graft,the nerve functions were tested at 1 year after the operation,all cases were followed up after 1 to 3 years.Results:In Group A、C,the number of regenerating fibers began to increase at 4 weeks,then transform nerve tractus a 24 weeks;and in Group B,it is lack of the regenerating fibers.The recovery rate of compound action potential peak and nerve conduction velocity is more quickly in Group A、C than that of Group B.In clinic,the excellence rate was 65.8% assessed by Seddon standard in all case.Conclusions:Fetal nerve combined with autogenous Schwann cell graft is a novel surgical technique in treating the peripheral nerve defects.
