摘要
目的:应用多排螺旋CT三期增强扫描,探讨结肠癌分级的可行性。方法:收集术后有病理结果的结肠癌病例57例,其中腺癌2级45例,腺癌3级12例。采用GE Discovery CT 750HD扫描仪。行平扫、动脉期、门静脉期、延时期增强扫描。分别测量各期CT值,进行统计学分析。结果:平扫腺癌2级与腺癌3级平均CT值基本相同。动脉期平均CT值腺癌2级略高于腺癌3级。门静脉期和延时期平均CT值腺癌2级略低于腺癌3级。两者门静脉期和延时期平均CT值均高于动脉期。三期增强CT值与主动脉CT值比较,动脉期差值腺癌2级小于腺癌3级,差异无统计学意义(P>0.05);门静脉期差值和延时期差值腺癌2级高于腺癌3级,差异无统计学意义(P>0.05)。结论:三期增强扫描对腺癌2级和3级的鉴别在门静脉期和延时期平均CT值得到证实,而在动脉期平均CT值比较时未得到证实。三期强化后CT值与主动脉CT值的差值比较时,未能得到统计学证实,有待于进一步完善研究方法。
Objective：To discuss the feasibility of colon cancer classification by applying three periods enhanced MSCT scan. Methods：57 cases of colon cancer with pathological results were collected,including 45 cases of levelⅡadenocarcinoma and 12 cases of levelⅢadenocarcinoma. GE Discovery CT 750HD scanner was used for line scan,arterial,portal venous phase scan and delayed scan. CT values of every period were measured and carried on the statistical analysis. Results：Average CT value of levelⅡ adenocarcinoma was basically similar to level Ⅲadenocarcinoma at the time of line scan and delayed period,but levelⅡwas slightly higher than the levelⅢat the time of arterial period;levelⅡwas slightly lower than the level Ⅲat the time of portal venous period and delayed period. Average CT value of them at the time of portal venous phase and delayed period were higher than arterial period. After compared enhanced CT value of three periods with aortic CT value,it turned out that average CT value difference of levelⅡadenocarcinoma was less than levelⅢ. There was not the statistically significant difference（ P﹥0 . 05 ）. LevelⅡadenocarcinoma was higher than levelⅢ at the time of portal venous and delayed period. There was not statistically significant difference too（ P﹥0 . 05 ）. Conclusion：Average CT value of portal venous phase and delayed period of LevelⅡand Ⅲadenocarcinoma is not significantly confirmed to distiguish them,while neither is the average CT value of arterial period and difference of every phase CT value and the aorta CT,which is waiting for further perfect inspection methods.
