摘要
目的:观察四肢深二度烧伤创面削痂术后应用重组人酸性成纤维细胞生长因子(recombinant human acidic fibroblast growth factor;rh-aFGF)的临床效果。方法:将46例四肢深二度创面的烧伤患者随机分为治疗组A(22例)和对照组B(16例)和对照组C(8例),治疗组A:患者削痂术后应用rh-aFGF,对照组B:患者削痂术后不用rh-aFGF,对照组C:患者削痂术后应用重组牛碱性成纤维细胞生长因子(bovine basic fibroblast growth factor,bFGF)。观察3组创面术后第10、14、18天的愈合率及平均愈合时间,治疗组的不良反应,随访愈合创面瘢痕增生的程度。结果:治疗组A、对照组B、C术后第10天愈合率分别为39.85%±12.01%,22.16%±8.86%,30.12%±8.87%。第14天愈合率分别为81.03%±16.81%,54.62±11.15%,66.21%±12.43%,3组比较显示治疗组A愈合速度更快,优于对照组B(P<0.05)及对照组C(P<0.05)。3组平均愈合时间分别为(14.41±2.28)d、(20.32±1.15)d、(17.76±2.14)d,因此治疗组愈合时间最短。治疗组A、对照组B、C术后中重度感染发生率分别为0,12.5%,37.5%。治疗组A水肿、渗出评分均明显低于对照组B、C(P<0.05),脓液评分3组并无统计学差异(P>0.05)治疗组A瘢痕量表评分、瘢痕面积、平均瘢痕高度分别为5.86±2.05,17.3±10.3,0.6±0.1,对照组B分别为8.12±1.86,33.7±19.5,1.7±0.4;对照组C分别为7.72±2.56,23.4±14.7,0.9±0.3。治疗组A与对照组B、对照组C比较均具有统计学差异(P<0.05)三组治疗过程中均未发生明显不良反应。结论:四肢深二度烧伤创面削痂术后应用rh-aFGF可以缩短愈合时间,减轻瘢痕增生,无不良反应。
Objective To observe the limbs tangential excision of deep second degree burn wounds after application of recombinant human acidic fibroblast growth factor （recombinant human acidic fibroblast growth factor; rh-aFGF） clinical results. Methods 46 cases of limb wounds deep second degree burn patients were randomly divided into treatment group A （22 patients） and control group B （16 patients） and control group C （8 cases）,the treatment group A：patients with postoperative tangential excision rh-aFGF,group B： tangential excision surgery in patients without rh-aFGF,group c： patients with postoperative tangential excision recombinant bovine basic fibroblast growth factor （bovine basic fibroblast growth factor, bFGF） observed three groups wound healing rates after the first day of 10,14,18 and average healing time,adverse reactions in the treatment group,the extent of follow-up wound healing scar formation. Results The treatment group A,group B,C after the first 10 days of healing rates were 39.85%±12.01%,22.16%± 8.86%,30.12%± 8.87%. The first 14 days of healing rates were 81.03%±16.81%, 54.62%± 11.15%,66.21%±12.43%,the three groups showed that the treatment group A healed faster than the control group B （P〈0.05） and the control group C （P〈 0.05）.Three average healing time was （14.41 ±2.28）d; （20.32±1.15）d; （17.76±2.14） d,so treatment group shortest healing time .
