摘要
目的：探讨鼻腔内翻性乳头状瘤患者行鼻内镜下低温等离子射频手术治疗的临床疗效。方法：选取我院2010年6月到2013年6月期间收治的35例鼻腔内翻性乳头状瘤患者,根据Krouse分级,其中I和II级患者单纯采用鼻内镜下低温等离子射频手术治疗,III级患者首先采用低温等离子射频将瘤体主体部分切除,在此基础上再对重要的瘤体累及部位行骨膜下切除。结果：手术时间在15-80 min,中位数手术时间为（40.6±7.3）min。I级和II级病变患者出血量为5-40 ml,III级病变患者为70-230ml,中位数出血量为（87.4±12.6）ml。所有患者术后疼痛轻微,鼻腔通气状况良好,于术后2-3d出院。所有患者均于术后2-3个月后见术腔全部上皮化,均无手术相关并发症发生,并连续随访半年无一例复发。结论：通过对NIP患者行鼻内镜下低温等离子射频手术治疗的疗效显著,具有手术野清晰,创伤性小,患者术中出血量少,手术时间短等优点,可作为一种微创手术治疗NIP的良好方法,对提高患者的生活质量具有重要意义。
Objective：To study of the effects of the low-temperature plasma radiofrequency ablation combined with endoscopy for the treatment of patients with nasal inverted papillomo. Methods：35 cases with nasal inverted papilloma from june 2010 to june 2013 were treated according to tumor stage by Krouse. Among them, patients in stage I and II were completed excised using the low-temperature plasma radiofrequency ablation combined with endoscopy, while patients in stage III firstly excised the main part of the tumor using the low-temperature plasma radiofrequency ablation combined with endoscopy, then subperiosteal resection of important parts of tumor invasion. Results：The Operative time were from 15 min to 80 min, with a median operative time was （40.6±7.3） min. Patients in stage I and II had the amount of blood loss from 5ml to 40ml and patients in stage III from 70ml-230 ml, with a median blood loss was （87.4±12.6） ml. Shorter operative time, minimal postoperative pains, epithelization of nasal cavity was seen 2 or 3 months postoperatively. There were no complications and after six months’ followed-up, none of patients had a recurrence. Conclution：The low-temperature plasma radiofrequency ablation combined with endoscopy is a minimally invasive treatment for patients with nasal inverted papillomo, with less blood loss, short surgery time, mild injury, mild postoperative pains. Thus it is important to improve the qualities of life for patients with nasal inverted papillomo.
