摘要
为了缩短Ⅲ、Ⅳ类手术患者术后留置导尿的时间,降低尿路感染的发生率,提高护理质量,运用品管圈开展质量改进。采用特性要因图分析Ⅲ、Ⅳ类手术患者术后留置导尿时间长的原因,采用因果关联分析及临床查检的方法找出真因。针对5个真因,制订改进方案。经临床实践证明:制订早期拔管的标准流程;对医护人员进行培训,制订新入科人员培训计划并实施;术前指导床上排尿;制作健康教育手册及宣传展板,能有效缩短Ⅲ、Ⅳ类手术患者术后留置导尿的时间,降低尿路感染的发生率,减轻患者的不适感,提早术后下床活动的时间,缩短平均住院日。
The quality control circle was implemented to shorten the duration of indwelling urinary catheter in postoperative patients undergoing type Ⅲ and type Ⅳ surgery,and then reduce the incidence of catheter-associated urinary tract infection and improve the quality of care.The measures included analyzing the possible reasons of prolonged duration of indwelling urinary catheters by using the characteristic reason diagram,finding out the real causes by using causation diagraph and clinical checklist,implementing improvement measures aimed at the five real causes,such as formulating standard flowchart of removing the urinary catheter early,making staff training plan,guiding patients to practice urination in bed before operation,designing health education manual and propaganda board.This practice effectively shortened the duration of indwelling urinary catheter,reduced the incidence of catheter-associated urinary tract infection,relieved discomfort and shortened the hospital stay in postoperative patients undergoing type Ⅲ and type Ⅳ surgery,
