摘要
目的探讨在新生儿肺炎的治疗过程中使用人免疫球蛋白(IVIG)对改善患儿身体状况的药力作用。方法选取2012年2月—2013年2月期间在保定市第一中心医院接受治疗的118例新生儿感染性肺炎患者,并将其均分为观察组和对照组。对观察组新生儿的治疗除进行常规抗感染等治疗外,再另加用静脉注射IVIG;对照组新生儿进行常规治疗。对两组新生儿进行疗效检测及观察治疗前后两组患儿T、B细胞增殖,血清中IgA、IgM、IgG、IgG亚类反应,外周血中单个核细胞产生白细胞介素-6(IL-6)、白细胞介素-4(IL-4)、白细胞介素-2(IL-2)及IgG亚类的能力。结果与对照组比较,治疗后血清中lgG及IgG亚类水平,观察组明显升高,外周血中单个核细胞产生IL-6、IL-4、IL-2及IgG亚类的能力无明显差别。观察组在体温恢复正常、咳嗽呼吸困难好转时间及肺部啰音消失时间方面花费的时间较对照组短,2组差异有统计学意义(P<0.01)。观察组总有效率为94.9%,对照组为71.2%,2组差异有统计学意义(P<0.01)。结论使用IVIG可增加患儿体内血清中IgG及IgG亚类水平,因此,针对新生儿肺炎患者在治疗过程中辅以静脉注射IVIG可提高其治疗过程中的药力作用。
[Objective]To explore the curative effect of human immunoglobulin(IVIG) in treatment of newborn pneumonia,study its function on improvement of physical condition in neonates.[Methods]118 patients with newborn pneumonia,who were treated in the First Central Hospital of Baoding City from February 2012 to February 2013,were equally divided into the observation group and the control group.The observation group was given the conventional anti-infection treatment and intravenous injection of IVIG,while the control group received the conventional treatment.The curative effects of two groups were detected.The proliferation of T and B cells,responses of serum IgA,IgM,IgG and IgG subclasses,and the ability of peripheral blood mononuclearcell to produce the IL-6,IL-4,IL-2 and IgG subclasses before and after treatment were observed in two groups.[Results]After treatment,levels of the serum IgG and IgG subclasses in the observation group were significantly higher than those in the control group,while there were no significant differences in the ability of peripheral blood mononuclearcell to produce the IL-6,IL-4,IL-2 and IgG subclasses.Compared with the control group,the time expending of temperature returned to normal,cough and dyspnea improvement,and pulmonary rales disappeared in the observation group were shorter,and the differences between two groups were significant(P〈 0.01).The total effective rate in the observation group and the control group was 94.9% and 71.2% respectively,and the difference was significant(P 〈 0.01).[Conclusion]IVIG can increase the level of serum IgG and IgG subclasses in neonate patients.Therefore,the adjunctive therapy of intravenous injection of IVIG can improve the curative effect in newborn pneumonia.
