摘要
目的探压封闭引流技术(vacuum sealing drainag,VSD)联合系统性湿性疗法治疗Ⅳ期压疮的效果,以期为寻找压疮防治的最佳方法提供依据。方法便利抽样法选择2011年1月至2012年6月在上海浦东新区周浦医院治疗的39例Ⅳ期压疮患者为研究对象,按入院先后将其分为A(n=19)组和B(n=20)组。两组患者均给予常规压疮护理,在此基础上A组患者给予系统性湿性疗法,按干燥坏死期、溃疡渗出期、肉芽组织期、上皮形成期给予轮替换药;B组在系统性湿性疗法基础上加用VSD治疗,即干燥坏死期、上皮形成期治疗同A组,溃疡渗出期、肉芽组织期改用VSD治疗。随访2个月,观察两组患者压疮渗液pH、创面愈合率、愈合时间及治愈率。结果两组治疗后压疮渗液pH均低于治疗前,差异有统计学意义(P<0.05)。A组压疮治愈率73.7%,平均治愈时间(36.6±12.5)d;B组治愈率85.0%,平均治愈时间(24.8±9.5)d;两组患者治愈时间的差异有统计学意义(P<0.05),创面愈合率的差异无统计学意义(P>0.05)。结论 VSD和系统性湿性疗法进行交替序贯治疗,可显著改善Ⅳ期压疮治疗效果,加速创面愈合。
Objective To explore the effectiveness of vacuum sealing drainag combined with systemic moist therapy for stage IV pressure ulcer, and to provide evidence for looking for better prevention method of pressure ulcer. Methods By convenience sampling, 39 patients with stage IV pressure ulcer were randomly divided into group A （n = 19）, and group B （n=20）. Two groups of patients were all had routine pressure ulcer nursing, and group A was treated with systemic moist therapy, that is, systemic dressing for dry necrosis, ulceration exudative phase and the period of granulation tissue, the epithelial formation. Group B was treated with vacuum sealing drainag combined with systemic moist therapy, that is, systemic dressing for dry necrosis and the epithelial formation, but vacuum sealing drainag for ulceration exudative phase and the period of granulation tissue. Following-up the two groups for 2 months about the size and depth of ulcer, change of pH value, the healing percentage, the healing rate and healing time were observed. Results pH value of two groups were all significantly decreased after treatment（P〈0.05）. The healing rate of group A was 73.7% and the average healing day was 36.6 ± 12.5. The healing rate of group B was 85.0% and the average healing day was 24.8±9.5. There was a significant difference in the average healing time（P〈0.05）, and there was no significant difference in the healing percentage（P〈0.05）. Conclusion Vacuum sealing drainag combined with moist therapy can effectively improve the treatment effect of IV stage pres- sure ulcer, and accelerate the wound healing.
