摘要
目的探讨老年2型糖尿病患者血糖和胰岛素抵抗对CA199水平的影响。方法选取2010年6月～2012年12月于广西医科大学第一附属医院收治的89例老年2型糖尿病患者（老年糖尿病组）和75例中年2型糖尿病患者（中年糖尿病组），两组再按照CA199水平分为CA199阴性组（CA199〈37U／mL）和CA199阳性组（CA199≥37U／mL）4个亚组，即老年糖尿病CA199阴性组（55例）、老年糖尿病CA199阳性组（34例）、中年糖尿病CA199阴性组（46例）、中年糖尿病CA199阳性组（29例）。比较各组空腹血糖（FBG）、餐后2小时血糖（2hPBG）、空腹胰岛素（FIns）、餐后2h胰岛素（2hIns）、糖化血红蛋白（HbA1c），CA199，胰岛素敏感指数（ISI）和胰岛素抵抗指数（HOMA—IR），将CA199水平与以上各参数进行Pearson相关分析。结果老年糖尿病组和中年糖尿病组中．CA199阳性组的FBG、2hPBG、Fins、2hIns和HOMA—IR均高于CA199阴性组，而ISI低于CA199阴性组，差异有统计学意义（P〈0．05）。老年糖尿病CA199阳性组的FBG、FIns、2hIns、HbAlc高于中年糖尿病CA199阳性组，差异有统计学意义（P〈0．05）。相关分析显示，中年糖尿病组的血清CA199水平与FBG、2hPBG、HbA1c、HOMA—IR呈正相关，与ISI呈负相关。老年糖尿病组的血清CA199水平与FBG、2hPBG、2hIns、HbA1c、HOMA—IR呈正相关，与ISI呈负相关。结论血清CA199升高与血糖代谢异常及胰岛素抵抗密切相关。
Objective To investigate the relations of CA199 level with blood glucose and insulin resistance in elderly patients with type 2 diabetes mellitus. Methods The 89 elderly inpatients and 75 middle-aged inpatients with type 2 diabetes in Division of Geriatrics Endocrinology, the First Affiliated Hospital of Guangxi Medical University from June 2010 to December 2012 were divided into elderly diabetes group （89 cases） and middle-aged diabetes group （75 cases）. And the two groups were divided into CA199 negative group （CA199 〈 37 U/mL） and CA199 positive group （CA199≥ 37 U/mL）, the four groups were elderly diabetes CA199 negative group （55 cases）, elderly diabetes CA199 positive group （34 caes）, middle-aged diabetes negative group （46 cases） and middle-aged diabetes positive group （19 cases）. The levels of Fasting blood glucose （FBG）, 2 hours postprandial blood glucose （2hPBG）, fasting insulin （Fins）, 2 hours postprandial insulin （2hIns）, glycosylated hemoglobin （HbA1c）, CA199, insulin sensitivity index （ISI） and insulin resis- tance index （HOMA-IR） between the four groups were compared. The Pearson correlation analysis was carried out be- tween CA199 level and above parameters. Results In elderly diabetes group and middle-aged diabetes group, the lev- els of FBG, 2hPBG, Fins, 2hIns and HOMA-IR in the CA199 positive groups were significantly higher than those in the CA199 negative groups, but the ISI level was lower than that in the CA199 negative groups, the differences were statistically significant （P 〈 0.05）. In the elderly diabetes CA199 positive group, the levels of FBG, Fins, 2hlns and HbA1c, were higher than those in middle-aged diabetes CA199 negative group, the differences were statistically signifi- cant （P 〈 0.05）. The correlation analysis showed that, in middle-aged diabetes group, the CA199 level was significantlypositive correlated with FBG, 2hPBG, HbA1c and HOMA- IR, the CA199 level was negative correlation with ISI; In elderly
