同步间歇指令通气辅以压力支持对慢性阻塞性肺疾病的疗效研究 被引量:2

摘要 目的观察同步间歇指令通气(SIMV)联合压力支持(PS)对慢性阻塞性肺疾病(COPD)的临床疗效,同时探讨SIMV+PS的临床意义。方法将该院呼吸内科收治的COPD患者92例随机分为两组,两组患者入院后均进行感染治疗、原发病治疗、营养治疗等常规疗法;对照组(42例)在此基础上行SIMV,观察组(50例)行SIMV+PS。结果两组患者呼吸频率、潮气量、每分钟通气量、吸气峰压(PIP)、动脉血二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)、动脉血氧饱和度(SaO2)、pH值等比较,差异均无统计学意义(P>0.05);观察组总通气时间、住ICU时间及脱机时间分别为(13±9)d、(15±10)d、(60±5)h,均明显短于对照组的(21±11)d、(22±8)d、(73±8)h,差异有统计学意义(P<0.05);观察组脱机成功率为92.0%,明显高于对照组的73.8%(P<0.05)。结论 SIMV+PS通气模式可加速脱机,减少COPD患者对呼吸机的依赖,有利于患者疾病愈合。 Objective To observe the effect of synchronized intermittent mandatory ventilation (SIMV) combined with pressure support(PS) in chronic obstructive pulmonary disease(COPD) .Methods 92 cases of patients with COPD were randomly divided into two groups .Two groups of patients were given infection therapy ,the primary disease therapy ,nutritional therapy and other conventional therapy .The control group(42 cases) underwent SIMV , the observation group(50 cases) underwent SIMV + PS .Results The difference of two groups of respiratory rate , tidal volume ,minute ventilation ,peak inspiratory pressure(PIP) ,arterial carbon dioxide partial pressure(PaCO2 ) ,arterial oxygen pressure (PaO2 ) ,arterial oxygen saturation (SaO2 ) ,pH value was not statistically significant ( P〈0 .05) .Total ventilation time ,ICU stay and offline time in observation group was (13 ± 9)d ,(15 ± 10)d ,(60 ± 5)h , significantly shorter than those of the control group of (21 ± 11)d ,(22 ± 8)d ,(73 ± 8)h (P〉 0 .05) .Observation group offline success rate was 92 .0% ,significantly higher than the 73 .8% in control group(P〉0 .05) .Conclusion SIMV + PS ventilation mode can accelerate offline ,reducing COPD patients on ventilator dependency ,which will help the patient a speedy recovery .
