摘要
目的探讨Ⅱ型糖尿病并发脑梗塞患者血小板GPⅥ的表达与血糖、血脂、Fib、hs-CRP以及Hcy之间的关系,以期明确糖尿病导致脑梗塞形成的机制。方法将260例脑梗塞患者分为单纯脑梗塞组121例和糖尿病合并脑梗塞组139例,另设健康对照组110例。测定各组血小板膜糖蛋白Ⅵ(GPⅥ)的表达以及血液中血糖(GLU)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、超敏C-反应蛋白(hs-CRP)、纤维蛋白原(Fib)以及同型半胱氨酸(Hcy)的含量。结果脑梗塞组患者血液中GLU、TC、TG、LDL-C、Fib、hs-CRP、Hcy、GPⅥ检测结果明显高于健康对照组,HDL-C检测结果明显低于健康对照组(均P<0.05);在脑梗塞患者中,糖尿病合并脑梗塞组血液中TC、TG、LDL-C、GLU、Fib、hs-CRP、Hcy、GPⅥ检测结果比单纯脑梗塞组明显增高,HDL-C检测结果明显减低(均P<0.05),且血小板GPⅥ表达水平与血液中Fib、hs-CRP、Hcy呈正相关;随着糖尿病病程的延长,血小板GPⅥ的表达和血清Hcy的含量明显增高(均P<0.05),血清中Fib和hs-CRP含量未见明显变化(P>0.05)。结论在糖尿病患者中,除了血糖和血脂的代谢紊乱外,血小板GPⅥ与炎症因子CRP、Hcy和Fib共同参与或介导了脑血管疾病的发生,对Ⅱ型糖尿病合并脑梗死的发生和发展起着重要的作用。
Objective To explore the relationship between type Ⅱ diabetes mellitus patients with acute cerebral infarction and the expression of GP Ⅵ blood glucose,blood lipid,Fib,hs-CRP and Hcy to clarify the mechanism of diabetes in the formation of cerebral infarction.Methods 260 cerebral infarction patients were divided into cerebral infarction group (121 cases) and diabetes complicated with cerebral infarction group (139 cases).110 healthy were as control group.The platelet membrane glycoprotein Ⅵ groups (GP Ⅵ) expression and blood glucose (GLU),total cholesterol (TC),triglyceride (TG),high density lipoprotein cholesterol (HDL-C),low density lipoprotein cholesterol (LDL-C),high sensitive C-reactive protein (hs-CRP),fibrinogen (Fib) and homocysteine (Hcy) levels were detected.Results GLU,TC,TG,LDL-C,Fib,hs-CRP,Hcy and GP Ⅵ of cerebral infarction group were significantly higher than those of healthy control group.HDL-C was significantly of cerebral infarction group was lower than that of healthy controls.TC,TG,LDL-C GLU,Fib,Hcy,hs-CRP and GP Ⅵ of diabetes complicated with cerebral infarction group were higher than that of cerebral infarction group (P〈0.05).The platelet GP expression of Fib,hs-CRP,Hcy Ⅵ levels were positively correlated with blood content.Conclusion In patients with diabetes,besides metabolic disorders of glucose and lipids,platelet GP Ⅵ and inflammatory cytokines CRP,Hcy and Fib participate in or mediate the occurrence of cerebrovascular disease,which play an important role in the genesis and development of type Ⅱ diabetes complicated with cerebral infarction.
