摘要 目的 比较NT-530P眼压计(NTT)测量的眼压值与Goldmann压平眼压计(GAT)测量眼压值的差异,评价NTT测量眼压的准确性及可重复性.方法 临床病例对照研究.选取2012年7~10月就诊于武警总医院眼科的50例患者和50名健康体检者共200只眼,分别使用NTT和GAT进行测量,比较二者眼压测量结果的重复性.使用配对t检验、相关分析等方法,比较二者测量结果的一致性.对患者分别行NTT及超声测厚法测量角膜厚度,经统计学检验后进行分析.结果 (1) NTF测量眼压的重复性:NTT组眼压读数的组内相关系数为0.913,差异有统计学意义(P<0.01),GAT组眼压读数的组内相关系数为0.894,差异有统计学意义(P<0.01).(2) NTT与GAT之间的关系:相关分析显示两种眼压计所测眼压值差异有统计学意义(r=0.718,P<0.01).NTT的测量值略低于GAT差异有统计学意义(t =4.113,P<0.01).(3)角膜厚度测量的准确性:经NTT所测得结果为(530.98±27.51) μm,超声测厚仪所得结果为(537.47±28.20)μm,行配对t检验差异有统计学意义(t=19.805,P<0.01).结论 NTT与GAT所测得的眼压值具有较好的相关性,其测量的重复性也均较好,而不同操作者测量的重复性则不完全一致.传统的超声测厚与NTT对角膜测厚结果有一定差异,并具统计学意义,但在临床意义上二者仍有较好的相符性. Objective To evaluate the reliability and reproducibility of measurement of intraocular pressure (TOP) with NT-530P tonometer (NTT).To investigate the clinical significance of corneal thickness measurement difference between NTT and corneal pachymeter.Methods Firstly,TOP was measured in 200 eyes of 100 cases using NTT and GAT.The reproducibility of IOP readings by NTT was evaluated.Secondly,the clinical data of comeal thickness were measured by ultrasonic corneal pachymeter and NTT were analyzed by statistical test.Results Intraclass correlation coefficient of the IOP readings obtained by NTT and GAT were 0.913 (P 〈0.01) and 0.894 (P 〈0.01),respectively,showing a good reproducibility in NTT and GAT measurement.IOP readings obtained by NTT and GAT presenting a good correlation between the 2 measurement methods (r =0.718,P 〈 0.01).However,a significant difference of IOP readings was found between the two methods (t =4.113,P 〈0.01).NTT underestimated the IOP in comparison with GAT.The thicknesses were (530.98±27.51)μtm and (537.47±28.20)μm from NTT and ultrasonic corneal pachymeter respectively,there was statistical significance by paired t-test.Conclusions The measuring results of NTT and GAT have preferable correlation.A good reproducibility is proved in both NTT and GAT methods.However,the reproducibility of the measurement by two different operators is not in accordance with each other.
