摘要 目的探讨MRI引导下高强度聚焦超声(HIFU)消融技术去犬肾交感神经化的可行性。方法健康杂种犬10只,随机均分为对照组和消融组。消融组于MRI引导下以HIFU点状消融右侧肾交感神经,声功率300W,辐照时间2s,能量600J;对照组实验流程相同,但不予消融。术后第6天检测动脉血压,之后处死动物,取右侧肾动脉和周围交感神经进行病理学检查,检测右肾实质去甲肾上腺素浓度评价去交感神经化效果。结果消融后第6天,消融组右侧肾动脉外膜周围交感神经所在区域出现凝固红色圆点,光镜下检查见神经纤维破坏,施万细胞损伤,对照组未见明显异常;两组动脉血压差异无统计学意义(P>0.05);消融组右侧肾实质去甲肾上腺素浓度显著低于对照组(P<0.05)。结论 MRI引导下HIFU消融技术可在犬体内有效实现肾去交感神经化。 Objective To investigate the feasibility of renal sympathetic denervation using MRI-guided high-intensity focused ultrasound (HIFU) in canines. Methods Ten healthy hybrid canines were randomly divided into control group and ablation group (each n=5). Punctate HIFU ablation with 300 W of power and 600 J of energy guided by MRI was performed on right renal nerves for 2 s in ablation group. Similar procedures without ablation were performed in control group. On the 6th day after ablation, blood pressure was measured, and then the animals were sacrificed, the right renal arteries and sympathetic nerves were observed pathologically. The norepinephrine concentration in the right renal parenchyma was measured to assess the efficacy of sympathetic denervation. Results In ablation group, red ablation lesions around the right renal arterial adventitia were observed with naked eyes, while disruption of nerve fibers and the injury of Schwann cells were observed under microscope on the 6th day after ablation. No obvious abnormity was detected in control group. There was no significant difference of blood pressure between the two groups (P〉0.05). The norepinephrine concentration in the right kidney parenchyma of ablation group was significantly lower than that of control group (P〈0. 05). Conclusion HIFU ablation guided by MRI is feasible for renal sympathetic denervation in canines.
