摘要
目的:分析右旋糖酐致死亡的原因,供临床安全用药参考。方法:检索中国知网、万方医学网、维普资讯网,以"右旋糖酐"、"死亡"、"致死"、"休克"、"过敏反应"、"严重"等为检索词检索截至2013年9月右旋糖酐致死亡的病例报告,分析其致患者死亡的原因。结果:共检索到右旋糖酐致死病例报告25篇(26例),其中19例患者死于过敏性休克。右旋糖酐致患者死亡的主要原因是与其他药物混合静脉滴注,特别是与复方丹参注射液的混合静脉滴注;与高致过敏性休克的药物先后静脉滴注是致死亡的次要原因,医务人员对患者过敏史和对患者发生药品不良反应的漠视也是重要的致死原因之一。结论:谨慎选择用药人群、严格控制用药适应证、单独静脉滴注、加强用药后30 min内的不良反应监测等是避免右旋糖酐致死亡的根本办法。
Objective：To analyze the cause of death induced by dextran so as to provide a reference for the safety of clinical medication. Methods：Through the retrieval of CNKI,WANFANG MED ONLINE and VIP Information Network and by key word searching to search the death case report caused by dextran up to September of 2013 and analyzed the cause of death of the patients. Results：About 26 death cases induced by dextran were reported in 25 articles,of which 19 patients died of anaphylactic shock. The main death cause of patients induced by dextran was the mixed intravenous drip of dextran with other medicines,especially with compound danshen injection. The secondary cause of death was the use of drugs which may induce anaphylactic shock in high risk before or after the intravenous drip of dextran. One of the other reasons was the disregard of patients’ history of allergies and adverse drug reactions by medical personnel. Conclusion： The fundamental approaches to avoid the death caused by dextran should be the careful selection of medication group of people , strict control of the drug indications, intravenous drip of single medicine and strengthening the monitoring of adverse reaction of dextran within 30 min after the drug administration.
