摘要
目的探讨肝脓肿患者腹腔感染病原菌及药敏分析,为临床合理使用抗菌药物提供依据。方法将2010年2月-2013年9月发生腹腔感染的74例肝脓肿患者为研究对象,应用ATB Expression全自动鉴定仪进行细菌鉴定,采用K-B法进行药敏试验,结果参照NCCLS药敏试验标准判定。结果 74例肝脓肿患者腹腔感染送检标本共分离出病原菌65株,其中革兰阴性杆菌54株占83.08%,革兰阳性球菌11株占16.92%;单一细菌感染51例占78.46%,合并两种病原菌感染7例占20.54%;大肠埃希菌对磺胺甲噁唑/甲氧苄啶、克林霉素、四环素、头孢噻吩、哌拉西林等药物耐药率分别为53.13%、50.00%、46.88%、43.75%、46.88%,克雷伯菌属对克林霉素、四环素、环丙沙星、头孢呋辛、阿莫西林的耐药率分别为77.78%、66.67%、55.56%、55.56%、55.56%。结论肝脓肿患者腹腔感染以革兰阴性菌为主,治疗时应进行药敏试验,根据药敏试验的结果合理使用抗菌药物。
OBJECTIVE To explore the pathogenic bacteria of abdominal infections in patients with liver abscess and drug sensitivity analysis, so as to provide the reference for clinical reasonable use of antibiotics for treatment. METHODS A total of 74 cases of hepatapostema patients with liver abscess infections from Feb. 2010 to Sep. 2013 were selected with pathogenic bacteria identification in ATB Expression auto system, and drug sensitivity test in K-B method referring to NCCLS. RESULTS In the total 74 cases of hepatapostema patients with liver abscess infections, 65 bacteria strains were isolated, among which gram negative bacteria was 54 strains (83. 08%) and gram positive bacteria were 11 strains(16.92%). A total of 51 cases in single bacterial infection, accounted for 78.46%. A total of 7 cases in two kinds of bacterial infection, accounted for 20. 54%. The drug resistance rates of Escherichia coli to sulfamethoxazole/trimethoprim, clindamycin, tetracycline, cephalothin, piperacillin were 53.13%, 50.00%, 46. 88%, 43. 75%, 46. 88%, respectively. The drug resistance rates of Klebsiella to clindamycin, tetracycline, ciprofloxacin, cefuroxime, amoxicillin were 77. 78%, 66. 67%, 55. 56%, 55. 56%, 55.56%, respectively. CONCLUSION Hepatapostema patients with liver abscess infection were mainly caused by gram negative bacteria, and drug sensitivity test should be performed during treatment, whose results should be based on when choosing antibiotics.
