The Application of Quantitative Parameters Obtained from DCE MRI in Evaluating Early Efficacy of Anti-Angiogenesis Drugs

摘要 目的探讨DCE-MRI技术中各定量参数无创性评价抗肿瘤血管药物早期疗效的应用价值。方法 30只结肠癌裸鼠皮下移植瘤模型分为安维汀注射液治疗组、5-Fu治疗组和对照组,每组各10只。药物治疗前及治疗后1 h、24 h、48 h行DCE-MRI检查,动态观察以下指标:微血管转运常数(Ktrans)、反流速率常数(Kep)、血浆容积分数(Vp)、血管外细胞外容积分数(Ve)及信号最大上升斜率(MSI),治疗后48 h MRI检查结束后处死载瘤裸鼠行免疫组织化学染色,上述MRI各定量参数与病理免疫组织化学结果行相关性分析。结果 Ktrans、Kep、MSI及Vp值在三组间具有统计学差异,且不同观察时相的变化趋势是不同的(P﹤0.01);Ve值在三组间无统计学差异(F=2.099,P=0.142)。两组药物治疗后1 h,各参数均明显下降;治疗后24 h,安维汀注射液药物组各参数持续下降,而5-Fu药物组有所恢复。Ktrans、Kep与免疫组织化学结果呈正相关,Ve、Vp及MSI与免疫组织化学染色之间无相关性。结论 DCE-MRI各定量参数可以反映药物的早期疗效,且能区分不同作用机制药物的早期反应,其中Ktrans、Kep可作为影像生物标记物定量无创性评估抗肿瘤血管药物治疗后肿瘤血管的关闭及细胞代谢情况。 Objective To discuss the application of quantitative parameters obtained from dynamic contrast enhanced (DCE) MRI in assessing the early effect of anti-angiogenesis drug. Methods Thirty rats with subcutaneous colon cancer xenograft model were randomly and equally divided into three groups with 10 rats in each group: the control group, the Avastin treatment group and the 5-Fu treatment group. DCE MRI was performed before the treatment (used as baseline), as well as at one, 24 and 48 hours after the treatment. The following parameters were dynamically measured: microvascular permeability transfer constant ( K^trans ), microvascular permeability reflux constant ( Kep ), volume fraction of the plasma space (Vp), extravascular extracellular volume fraction (Ve) and maximum slope of increase (MSI). After last MRI examination performed at 48 hours after the treatment, all the experimental rats were sacrificed and the specimens were sent for immunohistochemical examination. The DCE MRI parameters were quantified and correlated with immunohistochemical results. Results Statistically significant differences in K^trans, Kep, MSI and Vp values existed between each other among the three groups. In addition, the change trend was different in different observation phase (P 〈 0.01 ). No statistically significant difference in Ve existed between each other among the three groups (F = 2. 099, P = 0. 142). One hour after the treatment, all parameters in both Avastin group and 5-Fu group decreased significantly. At 24 hours after the treatment, all parameters in Avastin group showed a persistent dropping, while in 5-Fu group the parameters showed some improvement. K^trans, Kep had a parallel relationship with the immunohistochemical results, while no definite correlation existed between Ve, Vp, MSI and immunohistochemical results. Conclusion DCE MRI quantitative parameters can reflect the early effect of anti-angiogenesis drug and can also differentiate the early response of different drugs
