经鼻正压通气联合醒脑静治疗肺性脑病的临床研究 被引量:6

Clinical Study of Nasal Positive Pressure Ventilation Combined with Xingnaojing Treatment of Pulmonary Encephalopathy

摘要 目的探讨经鼻正压通气(BiPAP)联合醒脑静治疗肺性脑病的临床效果。方法选择2011年2月至2013年4月上海市黄浦区老西门街道社区卫生服务中心收治的100例肺性脑病患者为研究对象,依据住院号单双分成两组:对照组(48例)予以BiPAP治疗,观察组(52例)在对照组的基础上加用醒脑静治疗,比较两组治疗后的临床效果。结果治疗后,两组患者的PaO2高于治疗前(P<0.05),PaCO2、pH值和格拉斯哥评分均低于治疗前(P<0.05),且观察组改善程度较对照组更加显著(P<0.05);对照组总有效率为58.33%,观察组总有效率为92.31%,两组临床疗效比较,观察组显著优于对照组(P<0.05);观察组的面部压迫感、胃胀气、气胸发生率现象均低于对照组,两组比较差异有统计学意义(P<0.05)。结论 BiPAP联合醒脑静能改善肺性脑病患者临床显效,提高临床效果,降低不良反应发生率。 Objective To investigate the clinical effect of bi-level positive airway pressure(BiPAP) combined with Xingnaojing as treatment for pulmonary encephalopathy. Methods A total of 100 cases of pulmonary encephalopathy from Westgate Street Community Health Center of Huangpu District during Feb. 2011 and Apr. 2013 were included in the study,and were divided into two groups according to odd and even hospital numberof observation group(52 cases) and control group (48 cases):the control group was given BiPAP treatment,while the observation group was given BiPAP and Xingnaojing treatment, and the clinical effects of the two groups were compared. Results After treatment, PaO2 of both groups was higher than before treatment ( P 〈 0.05 ), PaCO2 , pH and Glasgow scores were lower than before treatment ( P 〈 0.05 ), and the improvement of the observation group was more obvious than the control group ( P 〈 0.05 ). Total effect of the control group was 58.33 %, of the observation group was 92.31%, and the difference was statistically significant ( P 〈 0.05 ). Adverse reactions including facial pressure, bloating, pneumothorax of the observation group were lower than the control group, and the difference was statistically significant( P 〈 0.05 ). Conclusion BiPAP combined with Xingnaojing can improve clinical effect of pulmonary encephalopathy, and reduce incidence of adverse reactions.
