2型糖尿病患者颈动脉弹性功能与血流最大剪切率及内-中膜厚度的相关性

Relationship among artery elasticity, maximal shear rate and intima-media thickness of carotid arteries in patients with type Ⅱ diabetes

摘要 目的探讨2型糖尿病患者颈动脉弹性功能、血流最大剪切率(SR)及内-中膜厚度(IMT)的相关性。方法 40例健康人作为正常对照,65例2型糖尿患者分为病程≤5年组(A组)和病程>5年组(B组)。利用高频超声对所有观察对象的颈动脉进行测量,包括颈动脉IMT、收缩期峰值流速(PSV)。利用血管回声跟踪(ET)技术测量相关参数,包括血管舒张期内径(Dd)、压力应变弹性系数(Ep),硬度指数(β)、顺应性(AC)。结果与对照组比较,糖尿病A组Ep、β增高(P<0.01),AC降低(P<0.05),IMT、PSV、Dd、SR差异无统计学意义;与糖尿病A组和对照组比较,糖尿病B组颈动脉IMT增厚(P<0.01),Dd增宽(P<0.01),PSV、SR降低(P<0.01),Ep、β增高(P<0.01),AC降低(P<0.05)。Ep、β与IMT呈正相关(r=0.527、0.556,P<0.01),与SR呈负相关(r=-0.513、-0.537,P<0.01),AC与IMT呈负相关(r=-0.477,P<0.01),与SR呈正相关(r=4.58,P<0.01),IMT与SR呈负相关(r=-0.537,P<0.01)。结论 2型糖尿病患者颈动脉弹性变化要早于IMT、SR改变,而低SR加速了血管内皮功能的损伤及动脉硬化的发展。 Objective To probe the relationship among artery elasticity, maximal shear rate (SR) and intima-media thickness (IMT) of the carotid arteries in the patients with type 2 diabetes. Methods 40 healthy volunteers were set as controls. 65 cases with type 2 diabetes were divided into two groups according to diabetes course: ≤5 years as group A and〉5 years as group B. In all subjects, the IMT, peak systolic velocity (PSV) of carotid arteries were measured with high resolution ultrasound. The diastolic diameter (Dd), pressure-strain elastic modulus (Ep), stiffness parameter(β) and arterial compliance (AC) were detected by echo-tracking (ET) technique. Results Compared with the control group, the Ep and β in group A were significantly higher (P〈0.01), and AC was lower (P〈0.05), while IMT, PSV, Dd, SR were not significantly different between the two groups. Compared with group B, the Ep, β, IMT and Dd were increased (P〈0.01), PSV and SR were decreased (P〈0.01), and AC was increased (P〈0.05). Ep andβshowed a significant positive correlation with IMT (r=0.527 and 0.556,P〈0.01) and a significant negative correlation with SR (r=-0.513 and -0.537,P〈0.01). AC showed a negative correlation with IMT(r=-0.477,P〈0.01) and a positive correlation with SR (r=4.58,P〈0.01). SR showed a negative correlation with IMT (r=-0.537,P〈0.01). Conclusion The change in arterial elasticity in the patients with type 2 diabetes appears earlier than IMT and SR. Lower SR could accelerate the injury of vascular endothelial function and the progress of arteriosclerosis.
