摘要
目的探讨应用超声心动图评价肺动脉压未升高的阻塞型睡眠呼吸暂停综合征(OSAHS)患者右心室功能的价值。方法正常对照组40例,OSAHS组60例均为肺动脉压未升高的患者,根据OSAHS的病情程度分为轻度16例、中度22例及重度22例。所有研究对象均应用超声心动图测量右心室结构和功能指标。结果 OSAHS各组患者右心室游离壁三尖瓣环处等容收缩期加速度(IVA)明显低于正常对照组(P<0.05),中、重度OSAHS组右心室IVA明显低于轻度OSAHS组(P<0.05)。中、重度OSAHS组右心室游离壁三尖瓣环处舒张早期峰值速度(Ea)、Ea与舒张晚期峰值速度(Aa)比值、等容收缩期峰值速度(IVV)均明显低于正常对照组(P<0.01)。中、重度OSAHS组右心室游离壁各节段收缩期峰值应变值均低于正常对照组(P<0.05)。结论中、重度OSAHS患者在肺动脉压未升高前就可出现右心室心肌运动障碍,组织多普勒和斑点追踪显像技术可敏感地检测出肺动脉压尚未升高前OSAHS患者右心室功能的改变,有重要临床应用价值。
Objective To evaluate the fight ventficular (RV) function of OSAHS patients with normal pulmonary pressure using echocardiography.Methods Forty normal subjects were enrolled as control group.Sixty patients OSAHS patients with normal pulmonary pressure were divided into three subgroups, including mild group (n=16), moderate group (n=22) and severe group (n=22).Conventional parameters of RV structure and function were obtained by two dimensional echoeardiography. Results The isovolumetrie systolic acceleration (IVA) of fight ventricle in the three OSAHS patients group were significantly lower compared with the control group (P 〈 0.05 ),the IVA in the moderate and severe group were significantly lower compared with that in the mild group (P〈 0.05 ).The Ea, ratio of Ea/Aa and isovolumetric systolic peak velocity in moderate and severe group were significantly lower than those in control group(P 〈 0.01 ).Conclusion The moderate and severe OSAHS patients with normal pulmonary pressure will show the dyskinesia of RV function.The tissue Doppler imaging and speckle tracking technologies can sensitively detect the change of RV function of OSAHS patients before the advent of pulmonary hypertension.
