钼靶X线及高频超声诊断触诊阴性乳腺病灶的比较分析 被引量:14

摘要 目的观察比较数字钼靶X线摄影和高频超声检查对触诊阴性乳腺癌病灶的诊断价值。方法回顾性分析82例触诊阴性乳腺病灶患者的钼靶X线及高频超声诊断资料,以病理诊断结果作为金标准,对两种影像学检查方法的诊断价值进行分析比较。结果本组82例患者82个触诊阴性乳腺病灶,术后病理诊断结果显示,28例患者阴性乳腺病灶为癌性病灶,均为单发。钼靶X线检查25例阳性,57例阴性,高频超声检查显示22例阳性,60例阴性。钼靶X线检查的敏感性高于超声检查,经比较差异有统计学意义(P<0.05)。钼靶X线检查的准确性、特异性虽高于超声检查,但经比较差异无统计学意义(P>0.05)。钼靶X线与高频超声诊断28个触诊阴性乳腺癌病灶的阳性率分别为82.14%(23/28)和64.29%(18/28),两组经比较差异有统计学意义(P<0.05)。两组均阳性的病灶占35.71%(10/28),仅钼靶X线显示阳性的病灶占46.43%(13/28),仅高频超声诊断显示阳性的病灶占28.57%(8/28)。钼靶X线检查乳腺癌病灶主要表现为恶性钙化,占诊断阳性病灶的69.57%(16/23)。高频超声检查乳腺癌病灶的超声特点主要为边界不清的低回声占位,占55.56%(10/18)。结论数字钼靶X线摄影和高频超声检查诊断触诊阴性乳腺病灶均有较高的临床应用价值,但钼靶X线具有更高的准确性、特异性及敏感性,具有更好的临床应用前景。 Objective To compare value of digital mammography and high frequency ultrasonography in diagnosis of nonpalpable breast lesions. Methods A retrospective analysis was made of the molybdenum target X-ray and high-frequency ultrasound diagnostic information of 82 cases diagnosed as nonpalpable breast lesions. Pathological diagnosis was taken as the gold standard to compare the diagnostic value of both imaging methods. Results Postoperative pathological diagnosis showed that 28 patients had solitary cancerous focus. Molybdenum target X-ray examination was positive in 25 cases,negative in 57 cases while high frequency ultrasonography showed 22 positive and 60 negative. The sensitivity of mammography was higher than ultrasound examination,with significant difference( P 〈 0. 05). Molybdenum target X-ray inspection is more accurate and specific than ultrasonography,yet with no significant difference( P〉 0. 05). The positive rates of mammography and ultrasound in diagnosis of 28 nonpalpable breast cancer were 82. 14%(23 /28) and 64. 29%(18 /28),respectively with significant differences( P 〈0. 05). The lesions diagnosed positive by both groups accounted for 35. 71%(10 /28),only by the molybdenum target X-ray 46. 43%(13 /28),only by high frequency ultrasound 28. 57%(8 /28). Mammography breast cancer lesions mainly showed malignant calcification,which accounted for 69. 57%(16 /23). The sonographic features of high frequency ultrasonography of breast cancer are ill-defined hypoechoic mass,which accounted for 55. 56%(10 /18). Conclusions Digital mammography X-ray photography and high-frequency ultrasound diagnosis of nonpalpable breast lesions have a high clinical value,but molybdenum target X-ray is more accurate,specific and sensitive,which has better clinical prospects.
