摘要 目的:了解机舱噪声对军事人员畸变产物耳声发射的影响及其与纯音听阈的关系。方法:采用纯音听力计和耳声发射测试仪,对40名空勤人员和30名地勤人员分别进行纯音听阈和畸变产物耳声发射检查。结果:两组受试者0.5-8kHz平均纯音听阈正常。两组受试者1-8kHz畸变产物耳声发射幅值正常,但空勤人员3、6和8kHz畸变产物耳声发射幅值低于地勤人员,两组差异具有统计学意义(3kHz,t=2.38,P〈0.05;6kHz,t=3.52,P〈0.01;8kHz,t=1.98,P〈0.05)。空勤人员高频纯音听阈异常率(22.5%)和地勤人员(13.3%)的差别无统计学意义(χ2=0.44,P〉0.05)。虽然空勤人员高频畸变产物耳声发射异常率(55.0%)高于地勤人员(33.3%),但两组间无统计学意义(χ2=2.43,P〉0.05)。结论:航空环境对空勤人员畸变产物耳声发射影响较纯音听阈大,军事飞行员有发生噪声性耳聋的可能性。 Objective:To investigate the effect of cabin noise on distortion product otoacoustic emission (DPOAE) of military personnel and their relationship with pure tone auditory threshold. Methods: The pure tone auditory threshold and amplitude of DPOAE were examined in 40 aircrew and 30 ground crewwith pure tone audiometer and otoacoustic emission analyzer. Results: The average level of pure tone auditory threshold at 0. 5- 8 kHz and amplitude of DPOAE at 1--8 kHz were normal in both groups. Amplitudes of DPOAE at 3, 6 and 8 kHz were significantly lower in aircrew than those in ground crew (3 kHz,t=2.38,P〈0.05;6 kHz,t=3.52,P〈0.01;8 kHz,t=1.98,P〈0.05). The abnormal rates of high frequency pure tone auditory threshold in aircrew (22.5%) and in ground (13.3%) were the same(X^2 =0. 44,P〉0. 05). Although abnormal rate of high frequency DPOAEW was higher in aircrew (55.0%) than that in ground crew (33. 3%), there was no statistical difference between them (X^2=.43, P〉 0.05). Conelusion: The effect of aerial environment on DPOAE amplitude is greater than that on pure tone auditory threshold in aircrew, and noise--induced deafness maybe occur in military pilots.
