摘要
目的胎儿生长部分取决于胎盘丰富血供。妊娠期糖尿病胎儿体重常增加明显,神经突起导向因子Netrin-1具有促血管新生作用,检测晚期正常妊娠及妊娠期糖尿病胎盘的微血管密度,同时测定相应胎盘表达Netrin-1的情况及患者娩出新生儿出生体重,从而探讨Netrin-1在妊娠期糖尿病中影响胎儿生长的可能机制。方法选取2012年1月1日—2013年9月12日间嘉兴市妇幼保健院剖宫产终止妊娠孕妇共46例,其中28例正常足月妊娠孕妇为对照组,18例妊娠期糖尿病患者为研究组。用免疫组织化学方法检测对所有入选病例胎盘中微血管数目;用实时定量PCR及蛋白免疫印迹检测所有入选病例胎盘Netrin-1表达情况,同时测定相应新生儿出生体重。结果在每高倍镜视野下正常足月妊娠及妊娠期糖尿病胎盘中,微血管密度依次为66.29±7.32、94.94±4.66。与正常足月妊娠相比,妊娠期糖尿病胎盘微血管密度增加明显(P<0.05)。正常足月妊娠及妊娠期糖尿病两组胎盘组织Netrin-1 mRNA含量依次为1.00±0.00及3.09±0.14,正常足月妊娠及妊娠期糖尿病两组胎盘组织Netrin-1蛋白含量依次为1.00±0.00及3.20±0.39。与正常足月妊娠相比,妊娠期糖尿病胎盘Netrin-1表达水平增加明显(P<0.05)。正常足月妊娠及妊娠期糖尿病两组新生儿出生体重依次为(3362.86±413.65)g、(3838.61±332.02)g。同时两组新生儿出生体重可见相同变化趋势(P<0.05)。结论 Netrin-1可能通过血管新生过程影响妊娠期糖尿病胎儿宫内生长过程。
Objective Fetal growth is partly determined by rich blood supply of placenta. And fetal weight in gestational diabetes mellitus often increase significantly, axon guidance factor Netrin-1 has a role in promoting angiogenesis. To ex- plore the role of Netrin-1 in fetal growth in pregnancies with gestational diabetes mellitus (GDM). Methods A total of 46 consecutive cases of pregnant women received cesarean section were analyzed from January 1 st,2012 to September 12th, 2013 in our hospital. 28 normal term pregnancy were served as control group, and 18 cases of GDM were as test group. The levels of microvessel density(MVD) and Netrin-1 in all placentas were detected, and neonatal birth weight was recorded at the same time. Results The MVD at higher power magnification in placentas of normal term pregnancy and gestational diabetes mellitus were 66.29 ± 7.32,94.94 ± 4.66. Compared with the MVD in placentas of normal term pregnancy,it is at higher power magnification in placentas of gestational diabetes mellitus( P 〈0.05 ). The leves of Netrin-1 mRNA in placentas of normal term pregnancy and gestational diabetes mellitus were 1.00 ± 0.00,3.09 ± 0.14. The leves of Netrin- 1 in placentas of the two groups were 1.00 ± 0.00,3.20 ± 0.39, the leves of Netrin-1 in placentas of gestational diabetes mellitusand were increase significantly than that in normal term pregnancy group. The neonatal birth weight in normal term pregnancy group and gestational diabetes mellitus group were were (3362.86 ± 413.65 ) g, ( 3838.61 ± 332.02 ) g, The same result of neonatal birth weight can be seen in the corresponding groups ( P 〈 0.05 ). Conclusion Netrin-1 may be involved in the control of angiogenesis and may play an important role in fetal growth of gestational diabetes mellitus.
