摘要
目的:探讨临床药师在查房中如何更好地开展药学服务。方法:遴选临床药师在查房中面临的13个案例,探讨临床药师在查房过程中如何以病人为中心进行更具体、完善的药学服务。结果与结论:临床药师在查房过程中,从用药教育、药物咨询、用药依从性、治疗效果、不良反应预防及监测等方面着眼工作,可以及时发现、预防、解决潜在的或实际存在的临床用药问题,提高药物治疗水平和医疗质量。
Objective： To explore better ways for pharmacists to improve pharmaceutical care in clinical rounds. Methods： Thirteen cases encountered by pharmacists in clinical rounds were chosen for in depth exploration of better ways for more spe- cific and comprehensive pharmaceutical care with patients as the focus. Results and Conclusion： Through efforts on drug educa tion and consultation, medication compliance, treatment efforts, adverse reactions prevention and medical monitoring, clinical pharmacists might be able to find, prevent and solve those potential or actual problems concerning clinical medication, which was surely beneficial to the improvement of drug treatment and medical quality.
