摘要
目的检测口腔鳞状细胞癌(OSCC)患者淋巴结标本中细胞角蛋白19(CK19)的表达情况,寻找发现OSCC淋巴结转移及微转移的有效分子生物学手段。方法应用荧光定量RT-PCR检测8例OSCC患者的119枚淋巴结标本中CK19mRNA表达水平,并与HE染色及免疫组化进行比较。结果 HE染色及免疫组化同时检测到CK19阳性淋巴结24枚,荧光定量RT-PCR检测到CK19阳性者48枚,HE染色及免疫组化结果CK19阴性而定量RT-PCR阳性的微转移淋巴结主要分布于颈Ⅱ区。淋巴结总体微转移率为20.17%。荧光定量RT-PCR检测的CK19阳性淋巴结与HE染色及免疫组化比较有统计学差异(x^2=22.04,P<0 01)。结论对于OSCC淋巴结转移与微转移的检测,荧光定量RT-PCR对CK19 mRNA的检测较HE染色和免疫组化敏感,分子病理检测方法的应用可以有助于肿瘤的分期判断、预后评估及治疗方案的抉择。
Objective To evaluate the expression of CK19 in lymph nodes from oral squamous cellcarcinoma and its clinical significance. Methods Expression of CK19 protein and mRNA was detected by immunohistochemistry and quantitative RT- PCR in 119 lymph nodes from 8 oral squamous cellcarcinoma patients, and its relation to the metastasis detected by HE was analyzed. Results In 119 lymph nodes from 8 oral squamous cellcarcinoma patients, 31 positive lymph nodes were detected by HE staining and immunohistochemical staining of CK19, and 55 by quantitative RT- PCR. Micrometasis was detected in 24 out 119 lymph nodes with a micrometastatic rate of 20.17%. Conclusion Detection of CK19 mRNA expression with fluorescent quantitative RT- PCR is more sensitive for diagnosis of metastatic and micrometastatic lymph nodes, which would contribute to the re- evaluation of tumor staging and treatment.
