摘要 目的探讨左心房功能指数(LAFI)在评价2型糖尿病患者左心房功能中的价值。资料与方法选择58例单纯2型糖尿病患者(不合并微血管病变的28例为A组,合并微血管病变的30例为B组)和30例健康志愿者(对照组),测量左心房前后径、左心室容积指标及室壁厚度,并计算左心室射血分数、心搏出量;测量左心室舒张早期峰值流速(E)、舒张晚期峰值流速(A)、二尖瓣环舒张早期运动速度(Ea)、舒张晚期运动速度(Aa),并计算E/A、Ea/Aa、E/Ea;于心尖五腔心测量左心室流出道血流的速度时间积分,用双平面法测量左心房最大容积、左心房搏出量、左心房射血分数,并计算LAFI。结果①与对照组比较,A组和B组LAFI均减小(P<0.05),B组LAFI较A组减小更明显(P<0.05);②LAFI与Ea、Ea/Aa呈正相关(r=0.460、0.475,P<0.01),与E/Ea呈负相关(r=-0.352,P<0.01),与Aa、E/A无显著相关性(r=0.003、0.069,P>0.05)。结论 LAFI可以用于评价2型糖尿病患者的左心房功能变化;LAFI与Ea、Ea/Aa、E/Ea有一定的相关性,可以间接反映左心室舒张功能的变化。 PurposeTo discuss the value of left atrial function index (LAFI) in theevaluation of left atrial function in patients with type 2 diabetes mellitus (T2DM).Materials and Methods Fifty-eight patients with T2DM (group A patients withoutmicroangiopathy, n=28; group B: patients accompanied with microangiopathy, n=30)and 30 healthy volunteers (control group) were enrolled in the study. The diameter ofleft atrium, left ventricular volume and wall thickness were measured and left ventricularejection fraction and stroke volume were calculated; mitral early diastolic and late diastolicpeak blood flow velocity (E and A, respectively), mitral annular motion velocity (Ea, Aa)and the outflow tract velocity time integral were measured for calculation of E/A, Ea/Aa,E/Ea. Maximal LA volume and left atrial ejection fraction were measured by using thebiplane method, together with the measurement of left atrial stoke volume, for calculationof LAFI. Results ① The LAFI in group A and group B was statistically lower than thatof control group (P〈0.05), and group B changed more dramatically than group A (P〈0.05).② LAFI was positively correlated with Ea and Ea/Aa (r=0.460, 0.475; P〈0.05) andnegatively correlated with E/Ea (r=-0.352, P〈0.01), and had no correlation with Aa or E/A (r=0.003, 0.069; P〉0.05). Conclusion LAFI can be regarded as a significant indicatorto evaluate left atrial function in patients with T2DM; LAFI is correlated with Ea, Ea/Aaand E/Ea, which can indirectly reflect left ventricular diastolic function.
