摘要
目的:通过随机对照试验(RCT)观察六味地黄丸联合银杏叶片对2型糖尿病患者趋化因子fractalkine的影响。方法:将116例2型糖尿病患者随机分为中药组与对照组,各58例。中药组加用六味地黄丸和银杏叶片,对照组加用安慰剂。连续观察24个月。结果:(1)干预24个月后,中药组肾小球清除率(GFR)呈上升趋势,颈动脉内膜中膜厚度(IMT)呈下降趋势。(2)尿微量白蛋白/肌酐之值(ACR)与中药干预、GFR、血脂、fractalkine、CRP有明显相关性,IMT与年龄、病程、CRP呈正相关(P<0.05)。回归分析显示,中药干预为糖尿病慢性并发症的保护性因素,fractalkine、CRP为其危险因素。(3)干预24个月后,中药组血清fractalkine由0.66(0.32,0.79)ng·ml-1下降至0.55(0.35,0.69)ng·ml-1(P<0.05)。结论:CRP、fractalkine为糖尿病慢性并发症的危险因素,六味地黄丸联合银杏叶片可有效减少fractalkine的释放,调节炎症通路,有效防治糖尿病慢性并发症。
Objective:To investigate the effect of combination therapy with Liuwei Dihuang plus Ginkgo Leaf on fractalkine in type 2 diabetic patients through RCT .Method:116 patients were randomly divided into treatment group(n=58) and control group(n=58), and treated with Liuwei Dihuang combined with Ginkgo Leaf or placebo respectively, and follow up with 24 months.Results:(1) After 24 months treatment, GFR of patients in treatment group showed an increasing tendency .IMT of patients in treatment group showed a decreasing tendency .(2) There was significant correlation between ACR and blood lipid , GFR, CRP, fractalkine, combination therapy with Liuwei&amp;nbsp;Dihuang plus Ginkgo Leaf .There was also significant correlation between IMT and age , duration of T2DM, CRP. The regression analysis showed combination therapy with Liuwei Dihuang plus Ginkgo Leaf was the protective factor of diabetic chronic complications , fractalkine and CRP were the risk factors .(3) The level of fractalkine in those who took Liuwei Dihuang combined with Ginkgo Leaf decreased from 0.66(0.32,0.79)ng· ml-1 to 0.55(0.35, 0.69)ng· ml -1(P&lt;0.05).Conclusion:Cytokines such as CRP and fractalkine were the risk factors of diabetic chronic complications .Combination therapy of Liuwei Dihuang and Ginkgo Leaf has efficacious treatment effect on diabetic chronic complications through inhibiting the release of inflammatory cytokine .
