摘要 目的通过研究针刺水沟穴对全脑缺血性昏迷大鼠海马区水通道蛋白-4(aquaporin-4,AQP-4)表达的变化,探讨针刺水沟穴对缺血性中风的治疗作用机理,为临床实践工作提供更加科学的实验依据。方法将100只SD大鼠,随机分为假手术组、模型组、醒脑静组、针刺水沟组和针刺水沟+醒脑静组,运用改良的四血管阻塞法建立全脑缺血性昏迷大鼠模型,对各组进行相应的干预,采用荧光定量RT-PCR法检测大鼠海马区AQP-4 mRNA的表达。结果模型组、醒脑静组、针刺水沟组、针刺水沟+醒脑静组的AQP-4 mRNA表达量较假手术组均增加(P<0.05),其中模型组显著增加,其次是醒脑静组、针刺水沟组次之,针刺水沟+醒脑静组增加不明显,各组间比较差异均具有统计学意义。结论 AQP-4可能参与了缺血性脑卒中脑水肿的发生、发展过程,针刺水沟穴配合药物可更有效的加强对AQP-4的抑制作用,减轻脑水肿,从而促醒缺血性昏迷大鼠。 Objective To study the change in aquaporin - 4 (AQP - 4) of brain ischemic coma rats treated with acupuncture at Shuigou(GV26) so as to discuss the effect mechanism in the treatment of ische- mic stroke treated with acupuncture at GV26 and provide the very scientific experimental evidences in clinical practice. Methods One hundred SD rats were randomized into a sham - operation group, a model group, a consciousness regaining group, an acupuncture group and an acupuncture + consciousness regaining group. The modified vessel occlusion method was used to set up brain ischemic coma rat model. The corresponding interventions were applied in the groups separately. The fluorescence quantitative RT - PCR method was used to determine the expression of AQP - 4 mRNA. Results The expressions of AQP - 4 mRNA in the model group,consciousness regaining group, acupuncture group and acupuncture + consciousness regaining group were increased as compared with the sham - operation group( P 〈 0.05 ) , in which, the expression in the mod- el group was increased significantly ,following by those in the consciousness regaining group and acupuncture group. The increase in the acupuncture + consciousness regaining group was not obvious. And the differences were not significant in comparison among the groups. Conclusion AQP -4 is probably involved in the oc- currence and progression of brain edema in ischemic stroke. Acupuncture at GV26 and medication can inten- sify much more effectively the inhibition to AQP -4 and alleviate brain edema so as to promote consciousness regaining in the rats of ischemic coma.
