摘要
目的探讨中国糖尿病患者胰岛素使用相关记忆问题(MPs)及其对患者生理和心理机能、工作和糖尿病管理的影响。方法对3个国家64例采用胰岛素治疗的患者进行焦点小组访谈和电话访问,根据结果设计网络调查问卷,在5个国家中开展网络调查,近1个月内与胰岛素使用相关的3种MPs,即忘记注射胰岛素(UF)、忘记是否注射胰岛素(QT)和忘记注射胰岛素剂量(QD)的发生频次、原因和后果。结果 354例完成调查的中国糖尿病患者中,74.6%患者报告有UF,83.1%患者报告有QT,65.8%患者报告有QD,42.9%患者报告3种均发生过。MPs对患者的生理和心理机能、工作时间均有负面影响,增加额外的血糖监测和医疗服务。结果在不同人口学特征、疾病特点和胰岛素治疗方案的患者中比较差异无统计学意义。结论与胰岛素使用相关MPs在中国糖尿病患者中非常普遍。MPs降低患者的总体健康状况和各项机能,占用额外的医疗服务资源,导致患者损失劳动时间,增加经济负担。
Objective To explore the memory problems （MPs） related to insulin-taking among people with diabetes in China, as well as the impact of such MPs on patients＇ physiological and emotional functions, work, and diabetes management. Methods 64 diabetics who in three countries treated with insulin have been researched on the frequency, reasons and outcomes of MPs by focus group to design a questionnaire. A web survey by the questionnaire was subsequently done in five countries. Three types of MPs associated with insulin-taking were examined, including unintentionally forgetting an insulin dose （UF）, questioning whether or not one took an insulin dose （QT）, and questioning how much insulin was taken （dose amount） （QD） over the past month. Results Among 354 Chinese patients who completed the web survey, 74.6% reported UF, 83.1% reported QT, and 65.8% reported QD. 42.9% of respondents reported experiencing all three of the MPs. MPs had negative impacts on patients physiological and emotional functions and work time. Moreover, it induced to extra blood glucose tests and medical care. There was limited variation in results across different demography, characteristics of diseases and insulin therapeutic regimens. Conclusion MPs related to insulin-taking among diabetics in China are common. MPs reduce the overall health and other physiological functions of patients, and increase waste of healthcare resources and loss of work time, then lead to corresponding economic effects.
