摘要
[目的]观察补阳还五汤联合偏瘫肢体运动疗法治疗中风后遗症疗效。[方法]使用随机平行对照方法,将52例门诊及住院患者按随机数字表法分为两组。偏瘫肢体运动疗法。对照组26例常规药物治疗:银杏叶提取物注射液、胞磷胆碱注射液、口服阿司匹林、阿托伐他汀等。治疗组26例补阳还五汤(生黄芪60g,归尾12g,川芎9g,赤芍10g,桃仁8g,红花6g,地龙10g,牛膝12g,续断10g,枸杞15g,桑枝8g);水煎200mL,1剂/d,早晚分服。口眼歪斜加僵蚕、全蝎、以祛风通络;言语不利加石菖蒲、远志以祛痰利窍;小便失禁加山萸肉、益智仁、五味子以温肾固涩;畏寒肢冷加肉桂、细辛以复振阳气,消除瘀血;肢体麻木加木瓜、陈皮、法半夏、茯苓以理气燥湿祛风痰、舒筋活络;下肢瘫软无力甚加桑寄生、鹿筋以补肾壮筋;上肢偏废加桂枝以通络;胸闷痰多或脘闷纳呆,舌淡苔腻加陈皮、半夏,以使痰去气畅;偏瘫日久加全蝎、土鳖虫以祛风通络;血压偏高加钩藤、石决明以平肝息风。连续治疗30d为1疗程。观测临床症状、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组痊愈7例,显效9例,有效9例,无效1例,总有效率96.15%。对照组痊愈5例,显效7例,有效8例,无效6例,总有效率76.92%。治疗组疗效优于对照组(P<0.05)。[结论]补阳还五汤联合偏瘫肢体运动疗法治疗中风后遗症效果显著,值得推广。
[ Objective ] To observe the Buyanghuanwu Decoction combined hemiplegic limbs movement therapy for stroke sequela curative effect. [ Methods ] Using random parallel control method, the 52 cases of outpatient and inpatient is divided into two groups according to random number table method. Hemiplegia physical exercise therapy. The control group 26 cases of conventional drug treatment： ginkgo biloba extract injection, citicoline injection, oral aspirin, atorvastatin, etc. Treatment group （ 26 cases for Yang also five soup （ Shenghuangqi 60g, Guiweil2g, Chuanqiong 9g, Chishao 10g, Taoren8g, Honghua 6g, Dilong 10g, Niuxi 12g, Xuduan 10g, Gouqi 15g, Sangzhi 8g;mouth eye askew and Jiangcan, Quanxie, with functions of t2dm;words add Shicangpu, Yuanzhi, adverse to the expectorant orifices;urinary incontinence and Shanyurou, Yizhiren, Wuweizi is acerbity;cold chills limbs with Rougui, Xixin to revive sun be the spirit, removing blood stasis, Mugua, Chenpi, Fabanxia, Fuling by facilitating dry wet wind phlegm, relaxes the muscles loose;lower limb paralysis and Sangjisheng, Lujin with kidney zhuang jin;upper limb while add Guizhi with t2dm, bosom is frowsty phlegm or often boring stay, pale tongue moss greasy and Chenpi, Banxia, to make the phlegm to qi chang;hemiplegia day Quanxie, Tubiechong of t2dm;high blood pressure and Gouteng, Shijueming with flat liver XiFeng ） . Boiling water 200 mL, 1 / d, points in the morning and evening. Continuous treatment of 30 d for 1 course of treatment. Observation of clinical symptoms and adverse reactions. Continuous treatment 2 period of treatment, judge curative effect. [ Results ] Treatment group recovered in 7 cases, 9 cases were markedly effective, effective 9 cases, 1 had no effect, the total effective rate was 96.15%. Control group healed in 5 eases, 7 eases had marked effect, 8 cases effect, 6 no effect, the total effective rate was 76.92%.
