摘要
目的 分析腹膜低度恶性囊性间皮瘤的病理形态学、诊断及鉴别诊断。方法 报道 18例腹膜低度恶性囊性间皮瘤 ,从病理大体及光镜、组织化学等方面进行观察。结果 18例腹膜低度恶性囊性间皮瘤 ,以中青年女性居多。肿瘤呈单发囊性或多发囊性肿块以及弥漫囊状小结节累及盆腔或腹腔腹膜。肿瘤微囊衬以单层立方状或扁平状细胞 ,瘤细胞有轻度异型性 ,其中 1例瘤组织转变成传统的恶性间皮瘤 ,3例瘤组织局灶性浸润肠壁深肌层 ,有 7例肿瘤复发。结论 提出此病的病理诊断要点 ,还对腹膜低度恶性囊性间皮瘤与腹膜恶性间皮瘤、囊性淋巴管瘤。
Objective To analyze the histopathological features of low grade malignant cystic mesothelioma of the peritoneum(LGMCMP). Methods Eighteen cases of LGMCMP were studied with gross and microscopic examination and histochemical staining. Results The majority of the patients were middle aged women.The tumors consisted of solitary and multiple cystic masses or diffuse cystic nodules involving in the pelvic and abdominal peritoneum.Microcystic walls were lined with cubic or flat cells of single layer with slight atypia.Three in 18 cases were found focal infiltration of the tumor in external muscular layer of the intestine,seven cases had recurrence,and one case finally developed into traditional malignant mesothelioma. Conclusions LGMCMP is a low grade malignant tumor with clinicopathologic characteristics.Its differential diagnoses include malignant mesothelioma,cystic lymphangioma,pseudomyxoma and adenomatoid tumors.
