摘要
目的:本研究通过观察动脉栓塞化疗(transcatheter arterial chemoembolization,TACE)结合三维适形放疗(three-dimensional conformal radiotherapy,3D-CRT)对中晚期肝癌患者的临床效果,探讨提高肝癌患者生存质量的有效方法,为后续临床研究提供参考。方法:选取2006年5月至2008年7月我院收治的55例不适宜手术治疗的中晚期肝癌患者为研究对象,采用肝动脉栓塞介入治疗与三维适形放疗相结合的方法进行治疗。观察患者治疗后的临床疗效、毒副反应发生情况等。结果:本组患者治疗的总有效率为74.6%;毒副反应主要表现为骨髓抑制、恶心呕吐及肝功能损害等,经对症治疗获得缓解;随访发现患者肿瘤局部复发率为22.7%,远处转移率为13.2%,5年生存率为68.8%,明显高于治疗前估值,差异具有统计学意义(P<0.05)。结论:在中晚期肝癌的临床治疗中,对患者实施放射治疗的同时选择合理的药物及给药方案进行化疗具有明显的近期疗效,患者的毒副反应可耐受,且治疗后的生存率较高,值得临床推广应用。
Objective:To discuss the best method of liver cancer by observing the clinical efficacy of the TACE combined with 3D-CRT on the treatment of metaphase and advanced hepatocellular carcinoma , to provide a reference for subsequent research .Methods:55 cases with metaphase and advanced liver cancer who were less proper to take the resections in our hospital from May 2006 to July 2008 were selected .The pa-tients were treated by the TACE combined with 3D-CRT.Then the clinical effects, the incidence of adverse reactions and the situation of follow-ups were observed and analyzed .Results:The total effective rate of pa-tients was 74.6%; The main adverse reactions were the myelosuppression , nausea and vomiting , and liver damage .of patients after the treatment and got the remission by symptomatic methods .It was found by follow-ups that the rate of recurrence was 22.7%, the rate of metastasis was 13.2%, the rate of five-years’ sur-vival was 68.8%which was higher than the prediction before the treatment with statistically significant differ-ence(P&lt;0.05).Conclusion:It suggests that the methods of TACE combined with 3D-CRT should be well promoted to the clinical treatment for patients with the metaphase or advanced liver cancer when applied with the reasonable medication and dosage regimen , because the clinical effects and the survival rate of patients are obviously higher , and the adverse reactions might be accepted .
